Overseas incremental credits for Medical and Dental staff will only be considered if this is their first
application in the NHS, and on completion of the required declaration form on appointment.

This must be completed and submitted within 6 weeks of starting in post otherwise the Trust reserves the
right to withdraw payment. The form is available from the Medical Staffing Department.

Failure to complete an application form will result in the Trust not being able to progress applications for
incremental credit on subsequent applications following appointment.

I wish consideration to be given to non-NHS service for incremental credit.
