Good morning everybody! My name is _________________, I am an
accredited assessor for _________________ NCII/NC III.
I would like acknowledge the presence of _____________, our representative
from TESDA Provincial Office. She/HE is here to oversee the conduct of
competency assessment and to check if the conduct is in accordance with the
prescribed competency assessment methodology and procedures. She will not
assess you.
Before I start the orientation, let me first check the attendance. As I call your
name, please affix your signature then present to me your respective
admission slip.

I have here with me your accomplished self-assessment guide and application
form. This is an indication that you know and can perform all the units of
competency enumerated in this qualification.
The purpose of our assessment for ______________ is to determine the level
of your competency, this can also be used for local or overseas employment
and promotion.
Today, I am going to administer competency assessment in _____________

conference room. And you can take your meal at the canteen located at the
lobby.
If you have difficulty in understanding the given instruction, please do not
hesitate to inform me because I am willing to translate it for you. But once the
assessment has started, please refrain from asking questions, it will no longer
be entertained.
During the actual assessment, please observe safety precautions as you
perform the different tasks. In case of accident, please call my attention
immediately.
With regard on the assessment result, we observe strict confidentiality.
In case you have any question regarding the assessment result, you have the
right to appeal. You can submit your appeal to the TESDA provincial Office

CAAS serves as an agreement between the candidate and the assessor. That
the assessor explained and the candidate understands the process and
requirements of the assessment.

 Will you please affix your signature in your rating sheet as well as in your
Competency Assessment Result Summary (CARS). The assessment center
manager will sign your CARS and will give it to you.

Before you go back to the holding area, I would like to inform you on how to
request or claim your National Certificate. In requesting your NC II that would
be from Monday-Friday 8:00AM to 5:00PM at TESDA Provincial Office located
at Bajada, Davao City.

Fill up the request form and attach the following: original copy of you CARS, 1
passport size picture with white background wearing collared garment. Then
fill up the request for payment and prepare Php 150.00, Php 50.00 for the
National Certificate then Php 50.00 for the Certification, Authentication and
Verification (CAV) and Php 50.00 for the documentary stamp.

Feedback for Not yet Ok ______________, based on the result of my observation during the oral
Competent questioning you did not answer all the question and you only got 2 correct
answer out of 10 questions while in the demonstration, in performing your
task you did not perform correctly in __________ and you have not finish your
task in a given allotted time and besides that you are not observing the safety
precaution. For that reason the outcome of your assessment is that you are
not yet competent. However, you can be re-assessed anytime at your
convenience or when you are confident to perform the given task which you
are found not yet competent. Do you have question?
