This document may be revised in order to address specific Project or location requirements in which
case it shall be approved by General Manager, MR or OHS Manager.

PROCEDURE
7.1 The Work Permit will be issued by the operations supervisor (Issuer).
7.2 The Work Permit will be received by the Permit Receiver only after he and the operation
supervision have inspected the site and reviewed the hazards involved.
7.3 No work may be commenced until a Work Permit has been issued applicable to that particular
operation and shift.
7.4 In the event of changing conditions creating hazards not previously specified, work must be
stopped and the Work Permit closed out. Work may not be resumed until a new permit has
been issued.
7.5 The Work Permit is a record of conditions and requirements and specifies the minimum
safety precautions which must be observed. They will respond to any request or warning
given by members of the operational staff and ensure that all personnel under their control
are aware of the evacuation procedures and assembly points.
Below are listed the basic rules which when implemented can assist in correctly
implementing the work permit system, however this is not meant to be a complete list and
additions shall be included in accordance with specific client requirements.
1. Issuer and Receiver shall inspect the site together prior to issuance of the permit.
2. Ensure the correct type and combinations of permit are issued (There may be a need to issue
more than one permit type).
3. Issuer and receiver must both be in possession of valid permit certification cards.
4. Job description and equipment must be clearly indicated on the permit.
5. Proper lockouts hold tags and blinds must be used where applicable.
6. Permit should be issued for specific time to cover the work.
7. Permits may be extended by oncoming shift issuer only when inspections etc are completed.
8. Special requirements such as fire watch or Scott Air Packs must be indicated on the permit.
9. The work permit must remain at the work location clearly visible and be closed immediately
should an emergency develop.
10. The work permit must be closed upon completion of the work, Issuer and receiver shall once
again inspect the area prior to closing the permit.
