( Nurse 1 call the patient to come in and asking health problem of the patient)

N1: Miss Suciati?

P : Yes, it's me. Good morning nurse

N1 : Good morning miss Suci. I nurse Firis and I am your nurse in this clinic. well miss Suci.
What’s your health problem miss?
P : I have cough for 3 weeks ago nurse
N1 : Do you have sputum in your cough?
P : Yes, nurse
N1 : Do you have blood in your cough?
P : Yes, I have

N1 : Okay, how is your appetite been like so far?

P : I have no good appetite

N1 : what makes you no willingness to eat?
P : I feel nausea
N1 : Could you eat your breakfast this morning? And how many portion did you eat for your
breakfast?

P : yes, I ate little this morning

N1 : do you lost your weight?
P : yes, before I sick my weight is 60 kg and now my weight is 53 kg
N1 : well, do you have fever ?
P : yes, I have fever. And I feel so cold and sweaty at night

N1 : okay, any another problem miss?

P : sometimes, I feel pain when I breathe or cough

N1 : Well, does anyone in your family have tuberculosis history?

P : yes, my mother had tuberculosis history 1 years ago

N1 : has she heal?
P : yes, she did

N1 : okay miss, now I would like to measure your vital signs

N1 : okay, I will measure your pulse and RR. So, breathe as normal

N1 : Your temperature is 37,8˚C and your RR and pulse is normal

Part 3 (nurse 2 report to doctor)

N2 : excuse me, dr. Desi there is a new patient. And her name is Suciati she is 22 years old

Dr : well and what’s her health problem?

N2 : her problem is she have cough for 3 weeks, there is sputum in her cough. And she have
bad appetite
Dr : how about her weight?

N2 : she lost 7 kg the body weight during she sick

Dr : okay, any another problem nurse?

N2 : she feel cold and sweaty at night and then sometimes, she feel pain when breathe or
cough. And her temperature is 37,8˚C, RR is 24x/mnt. That’s all doctor

Dr : okay, thank you nurse Putri. You can call the patient to here

N2 : sure, dr Desi

P : hallo doctor good morning
Dr : good morning, miss Suci
P : that’s right
Dr : okay, can you lay down on this bad, please?
Dr : I will examine your physical
P : sure
Dr : can you put off your clothes, please? Because I want to examination your chest and lungs
P : no problem
Dr: okay, take a deep breathe please? Does it pain?
P : yes

your skin. And this medicine to knowing do you have tuberculosis or not

P : yes please, but be carefull nurse

N2 : okay, now I will clean the injection site with alcohol swab. You may feel a little bit pain

N2 : okay, now I will draw a circle arround of the injection site

N2 : well, miss Suci you can come back 2-3 days to read the result
