TRANSPORTING FROM AREA TO AREA

1.1.2 Check wind speed is acceptable for lifting conditions

2. Transportation – use of trailer truck 2.1 Material fall 2.1.1 Secure the load, use binder or equivalent
2.1.2 Flag men are required, coordination with the truck
driver is necessary.

USE OF SCAFFOLDING AS WORKING FLATFORM

1. Scaffold erection 1.1 Unsafe scaffolding 1.1.1 Scaffold must be erect as per standard/ client
requirements
1.1.2 Scaffolding Foreman must prepare a STARRT Form
for more than 15 meters high

1.2 Hazard to fall 1.2.1 All Scaffolders should wear full body harness
equipped with two (2) LANYARD and SHOCK ABSORBER
(inspected and approved “color coded”)
Note:
Only with FPT (Fall Protection Training) stickers are
allowed to work above (2) two meters heights

2. Rebar fixing at heights 2.1 Hazard to fall due to open planks between 2.1.1 Wear full body harness (properly)
rebar and scaffold planks 2.1.2 Make sure that scaffold is safe to use and inspected
2.1.3 100% hooking is necessary
Note:
All designed erected scaffoldings are in yellow tag
which requires wearing full body harness.

WORKING INTO EXCAVATION

1. Working into excavated area 1.1. Injury due to not providing access 1.1.1. Provide good access where activity is to be done
1.1.2. Provide a clear walkways
1.1.3. Foreman should advise the workers to use the
access and not to jump into excavation
