BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS
1. Pre-Commencement Activities (Method Statement, 1.1Unsafe act and unsafe conditions due to failure 1.1.1 Fully planned with written step-by-step work activities

3. Erection of scaffold prior to removal of all over
head existing lights, cable tray and cables.
3.1 Fall accident (personnel & material),
respiratory problems, skin, eyes, nose, &
throat irritation
3.1.1 Pre inspection of the ladder and inspection of the
scaffold prior to use
3.1.2 Suitable Ladder will be provide (Inspected displaying
the current quarterly colour code and approved ladders)
Scaffolding will be used as the safe means of access
 3.1.3 PPE will worninspected and approved full body
safety harness with double lanyard and shock absorber
(displaying the current quarterly colour code), leather
gloves for handling electrical wires / removing utility
connection & respiration mask (dust mask) as protection
from respiratory hazards.

General Instruction:
All electrical cables / wires must be treated as live by all
electrical personnel doing disconnection works.

4. Use of Man Basket for removal of side cladding for 4.1 Hazard to fall, Defective equipment, 4.1.1 Crane daily inspection shall be carried out by the

basket
4.2.5 Cold Work Permit to be raised before commencing
activities

General Instruction:
No third party inspection do not use.

5. Erection of scaffolding prior to Installation of Life
lines
5.1 Fall accident (personnel & material)
5.1.1 Provide good access and egress by erection of
scaffolding
5.1.2 Scaffold must be inspected and approved.
5.1.3 Provide life lines (19 mm diameter) (green
tag/approval inspection), correctly installed
5.1.4 All materials and tools to be use at heights shall be
secured
5.1.5 Provide Full Body Safety Harness with double
lanyards and shock absorber (quarterly colour
coded/inspected)

General Instruction:
- Red tag scaffolddshall not be used
- Red tag life linedt shall not be used
6. Removal of cladding/ corrugated sheet and lay
down with crane
7. Removal of roof beam/ structure use of oxy-acetylene

commencing any cutting activity

between the crane operator and rigger to be maintained, ,
use of an additional rigger if two (2) cranes are used.
6.1.5 Beams to be slung up and attached to the second
crane prior to the removal of bolts/nuts from the beams
6.1.6 Use tag line to handle the material and man basket
6.1.7 Check wind speed is acceptable for lifting conditions,
if wind speed increases, lifting operations shall be
suspended
6.1.8 Position the crane in order not to strike adjacent
buildings / equipment
6.1.9Safe working load of the crane to be observed at all
times
6.1.10 Use of colour coded & inspected lifting equipment
6.1.11 Work area to be barricaded and warning signs

11. Mobilize tools and equipment to worksite 11.1 Traffic and pedestrians
8.2.1 Re-inspection of backhoe and mounted hammer
8.2.2 Operator Check-list for day-to-day work is necessary.
8.2.3 Unauthorized person is not allowed at the perimeter
work area. Barricade to be installed and watchman
positioned to prevent unauthorized personnel from
entering the area
8.3.1. All equipment shall be inspected prior to entrance to
site by the company and TAL HSE Dept
8.4.1 TBT and STARRT to be conducted prior to
commencing work
8.4.2. JSA’s Shall be discussed during the daily TBT
(STARRT)
9.1.1 Only qualified operator is allowed to operate
equipment.
9.1.2 Dust control by spraying water at the location.
9.1.3 Use of required PPE’ and respiratory mask is
mandatory
9.1.4 Restricted access to the area (barricade and
signboards)
9.1.5 Banks-men or spotter is required for the area.
10.1.1 All personnel to be clear of the area while
demolishion of the walls are in progress
10.1.2 Barricades and warning signs to be erected to
keep all unauthorized personnel out of the area
10.1.3 Designated spotter, banks-man for the area is
necessary
11.1.1. Audible reversing alarms shall be installed on
equipment/vehicles
11.1.2.
11.1.3. All traffic rules and signs should be followed (on site
speed limit is 25-km/h)
11.1.4. Only approved routes shall be used for access to
and egress from the site
11.1.5. The drivers shall be made aware of the approved
routes
11.1.6. Trained banks-men with high-visibility vests shall
accompany the work activities
 11.2 Overturning of vehicle / Crash injuries 11.2.1. All drivers shall have valid driving licenses, all
operators shall have 3rd party certifications
11.2.2. The areas where heavy equipment are working
shall be clearly marked on site
11.2.3. Safe access shall be maintained at site for vehicles
and equipment
11.2.4. Use of seatbelts shall be mandatory at all times

12. Loading, transportation(use of Loader and Truck) 12.1. Material falling from trucks
12.1.1. All materials on the trucks should be effectively
secured during transportation
12.1.2. Operators and Supervisor shall ensure that the
vehicle is not overloaded
12.1.3. All trucks shall be equipped with covering canvas
with ropes for easy (un)covering
12.1.4 All demolished materials will be transported to
designated areas as per Local Authority requirements .

12.2. Speeding 12.2.1. Operators shall be made aware of traffic rules and
regulations; penalty system shall be implemented if
required

12.3. Operator struck by the bucket (loader) 12.3.1. Truck driver shall leave his vehicle and stand at a
safe distance during loading

13. Housekeeping 13.1 Falling Objects, tripping hazards, skin 13.1.1 Use mobile crane to lift heavy objects
irritation, environmental hazard, hand injuries, 13.1.2 Complete PPE’ will be supplied and strictly enforced
back injury during lifting of heavy objects especially heavy duty Hand gloves, respiratory mask. (dust

13.1.3 Safety watcher will be assigned to oversee

14.2. Clutter/Rubbish 14.2.1. Good housekeeping practice shall be implemented
throughout the day
