a. Are there procedures or work instructions in place to cover the next operation Cranes
and forklift Rig devices such as crown block and travelling block, drilling line, raising
line, cathead, cat line, wire sling, hook shackle, air (electrical) tugger (winch), crane,
forklift and fitting, cathead , cat line, wire sling, hook shackle, air (electrical) tugger
(winch) , crane, forklift and fitting?
b. Are all lifting operation responsibilities correctly addressed at the lifting operation
procedures?.
c. Are all lifting operation responsibilities correctly known by rig crew?.
d. Are all lifting gear (Shackles, Web slings, wire sling, Chain block, etc.) in good
condition?

h. Has a new color introduced every six months and each color shall be current for a
period of 8 months(two months overlap)?

k. Do all personnel involved in the operation and /or maintenance of lifting equipment have
sufficient qualification and /or certification in addition to experience?

n. Have all rig crew members received basic rigging and slinging in house training ?
o. Have all lifting equipment used in drilling operation on site registered with all the
following relevant information of the lifting equipment: Manufacturer, Description,
Identification number and Safe working load?

p. Have all lifting Equipment (Accessories/Appliances) examined by third party and records
maintained by the Rig Manager & Safety as follows:
- Mobile Crane and crane wires (annually)
- Forklift (annually)
- Container lifting points (MPI every 6 months required and annual load test)
- Personnel baskets (6 months MPI required and annual load test)
- Web Slings (6 months/Visual)
- Shackles (6 months/Visual)
- Winches (6 months MPI required and annual load test)
q. Have all lifting operations by crane or forklift done only after submitting a PTW ?.
r. Has any lifting operation at rig site associated with risk assessment and Lift Plan ?
s. Do crane and forklift operators have valid license through Iraqi governorate and training
certificates by third party?
t. Are the crane and forklift in good condition and being inspected on daily basis by Forklift
and crane operators? Are the records in place?
u. Are the lifting accessories being inspected by Mechanic, Electrician and Safety supervisor
on weekly basis and records are in place? C3
v. Are Crane and forklift clearly addressed at Rig Site Equipment preventive maintenance
system? C4
w. Have all lifting operations (routine or non-routine) clearly specified at PTW system?
x. Are there any homemade lifting appliances at rig site?
y. Has the rig crew addressed a dedicated area for junk or defected lifting accessories
(Quarantine area)? C5
