MINUTES OF SLAC SESSION

PRELIMINARIES

Prior to the actual review of the SIP, orientation on the purpose of the meeting was done. The roles of
each of the members of the team were discussed by the chairman.

MEETING PROPER:

The meeting was called at 8:00 in the morning last April 10, 2019 at the school computer laboratory. The
chairman, Victorio A. Beltran formally opened the proceedings. It started with a prayer led by Mr. Ramel A.
Baluya. Roll call was done and a quorum was established with all members present:

During the meeting the following topics were discussed:

3. Child Friendly School Survey
4. Child Protection Policy
5. Student led School Watching and Hazard Mapping
6. Identifying Priority Improvement Areas

In the afternoon session, workshop on the review of the SIP was using the following worksheets:

2. Guidelines in listening to the Voice of the Learners and other Stakeholders
3. Walk the Process Guidelines
 4. Root Cause Analysis Overview
5. Project Work Plan and Budget Matrix
6. Annual Implementation PlanTemplate
