AFFIDAVIT

I, JERRY JOSEPH A. LIM, Filipino, of legal age, married and a resident of Taloto
District, Tagbilaran City, Bohol, Philippines, after having been duly sworn to in accordance with
law, hereby depose and state that:

1. I am the owner/proprietor of BOHOL SIGHTS AND TOURS;

2. Sometime in 2012, I hired an employee on probationary status; which I had registered

with the PAG-IBIG Fund in compliance with the law. The employee was then
terminated after three (3) weeks for not having passed the specific qualifications
needed for the position. Unfortunately, by my unintentional oversight, I was not able
to report said termination to the PAG-IBIG Fund Office;

3. After the termination of the employee, I had decided to run the business on my own
and had not hired any employee since. I had not remitted any amount to the PAG-
IBIG Fund relate to my contributions under BOHOL SIGHTS AND TOURS as I
have been already complying with the same being employed by another entity – La
Roca Group Inc.;

4. In 2016, due to financial and management problems, I had requested for the
temporary closure or suspension of the operations of BOHOL SIGHTS AND TOURS
in order to be exempted from paying any fees relative to the application of a business
permit from the City. A copy of the notice of temporary closure or suspension issued
by the City of Tagbilaran is herein attached as Annex “A” hereof;

5. The temporary closure or suspension remains effective today;

6. Based on the foregoing explanation, there is therefore no record of BOHOL SIGHTS

AND TOURS making any contributions from July 2012 to September 2018. This is
in relation to the notice and demand letter I had received on November 14, 2018 from
the PAG-IBIG Fund Office – Tagbilaran Branch. A copy of the notice and demand
letter is herein attached as Annex “B” and Annex “C” hereof;

IN WITNESS WHEREOF, I have set my hand this November 26, 2018 at Tagbilaran
City, Bohol, Philippines.
