AFFIDAVIT OF NO GRADE

I, PRESCIOUS T. ADAYA, Filipino, of legal age, single and a resident of

Malinao East, Lila, Bohol, Philippines, after having been duly sworn to in
accordance with law, hereby depose and say:

l. That I am a student of Ms. Gloria Leodivica I. Araneta (Ms. Araneta) for

English 3 (Speech Communication) last school year of 2017-2018 at University of
Bohol, Tagbilaran City, Bohol;

2. Unfortunately, Ms. Araneta gave me No Grade on such subject due to my

failure to submit my admission slip during our final exams and also due to
financial constraints;

3. That I would like to humbly ask Ms. Araneta to change my grade from
NG-LR to ______;

4. That I am executing this affidavit to attest to the truth of the foregoing

facts and for whatever legal purpose that this may serve.
