BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS

be clearly marked on site
2.3.3. Safe access shall be maintained at site for vehicles
and equipment
2.3.4. Use of seatbelts shall be mandatory at all times

2.4. Slips / Trips / Falls 2.4.1. Housekeeping shall be discussed in daily TBT
(STARRT) and be implemented on site on a daily basis
2.5 Environmental Hazard 2.5.1 All Environmental spills must report to HSE
Department
2.5.2 All Equipment and tools must be inspected and
approved

equipment that displays monthly/quarterly colour code
3.2.3 Only trained riggers, well identified ,(high visibility
vests), use tag lines to control the load
3.2.4 Crane lifting area to be barricaded. No unauthorized
entry into the area.
General Instruction:
- No defective crane should be in use
- No standing under the load

3.3 Environmental and health hazard due to spill of 3.3.1 Crane daily inspection (check list) is required
diesel, oil, fluids. 3.3.2 All Environmental Spill must be reported immediately
to GAMA-HSE Department
General Instruction:
Defective Mobile cranes should not be in use
3.4. Slips / Trips / Falls 3.4.1. Housekeeping shall be discussed in daily TBT
(STARRT) and be implemented on site on a daily basis
3.4.2 Working platform should provide before working
condition/area change, or if there are possibility to fall
3.4.3.Fall Prevention to be provided when working within 2
m of an excavation which exceeds 2m in depth
3.4.4 Full body harness with double lanyard and shock
absorber shall be worn

3.5. Injuries - caused by excavation collapse 3.5.1. First Aid kits and certified First Aiders shall be
available on site Clinic
3.5.2. All employees shall be made aware of Emergency
Procedures
3.5.3. Audible reversing alarms shall be installed on each
vehicle
3.5.4 All equipment to be kept away from the edge of the
excavation
3.5.5. Entry to the excavation area, which shall be properly
barricaded, shall be prevented when Inserting the
dewatering Pipe
3.5.6. All material should be at least one (1) meter away of
the excavation
3.5.5 Competent Person to inspect the excavation(s) prior
to work activities commencing, this to include after lunch
3.5.7. Side slopes of excavations be stable

3.6. Dust Control 3.6.1. Provision of dust masks for workers and ensure
their use
3.6.2. Provide dust mitigation by spreading water
3.6.3. The distance of water truck from excavation should
be at least one (1) meter away

4. Running of Dewatering Pump (24 hrs) 4.1 Human injury due to faulty equipment 4.1.1 All equipment must be inspected and approved
4.1.2Ensure that motor pump (rotating parts) are protected
guarded
4.1.3 No damaged equipment is allowed on t site
4.1.4 Provide Fire Extinguisher (9kg)
(same comment 3.2.1 to 3.2.4)
4.1.5 Provide Barricade(s) and signboard
4.1.6 Only qualified mechanic are allowed to do any
adjustment
4.1.7 Provide sufficient illumination for work out with the
normal working hours
4.2 Environmental and health hazard due to spill of 4.2.1 Daily inspection is required
diesel, oil, fluids. 4.2.2 All Environmental Spill must be reported
immediately to GAMA-HSE Department
4.2.3 Use of metal funnel in refilling of oil and diesel
4.2.4 Fuel truck to ensure earthing wire used when
refueling equipment.
General Instruction:
Defective motor pump should not be in use

8. Demobilization 5.1. Work completion 5.1.1. Workforce shall be alerted about the work
completion
5.2. Clutter/Rubbish 5.2.1. Good housekeeping practice shall be implemented
throughout the day
