A 24-year-old woman was taken to the hospital by her family with shortness of breath
a day ago due to fatigue. At the emergency department data obtained that the patient's
shortness of breath has been reduced but the patient still feels weak.
The next day in the patient’s ward.
Nurse : Good morning Miss
Patient : Good morning
Nurse : Hi I’m Rofi’i. I’m a registered nurse, and I’ll be looking after you today.
Could you tell me your name and your date of birth ?
Patient : My name is Nindy Arum, I was born November 8, 1993.
Nurse : Ok Miss Nindy Arum you were born November 8, 1993. And what would
you like us to call you ? Miss Nindy or Arum ?
Patient : Just call me Miss Nindy.
Nurse : Alright miss. Are you here alone or with your family ?
Patient : I’m here with my mom but she's leaving for a while.
Nurse : Ok then. I will check your vital signs that aim to determine your health status
and I took about 10 minutes. Are you willing?
Patient : Yeah let’s start then.
Then the nurse checks patient’s vital signs.
Patient : How is the result nurse, is it normal ?
Nurse : Yeah it’s normal. The results is your blood pressure is 120/70 mmHg, your
pulse is 95x/minutes, your temperature is 36,5oC, and your respiratory rate is
37x/minutes.
Patient : Thank you Nurse
Nurse : You’re welcome Miss. So the examination is complete, thank you for your
cooperation. Later I’ll come back to train you and your family how to prevent
shortness of breath with deep breathing techniques.
Patient : Ok sounds great.
In the noon
Nurse : Hello miss how are you feeling right now ?
Patient : I’m feeling better right now
Nurse : Good. According to my promise I will train you and your family how to
handle it with deep breathing techniques. I need about 15 minutes, are you
willing?
Patient : Yes I am willing
Nurse : Ok previously, I’m Rofi’i. I’m a registered nurse, and I’ll be looking after
you daughter today. So you’re the mother of Miss Nindy right ?
Family : Oh hi Rofi’i, yes, I’am the mother of Nindy. My name is Ella
Nurse : Hi Mrs. Ella nice to see you
Family : Nice to see you too
Nurse : Mrs. Ella what do you usually do when your daughter complains of shortness
of breath?
Family : I immediately took her to a hospital or health facility.
Nurse : Ok actually your actions are correct Mrs. Ella but if your daughter complains
of shortness of breath again give first aid when at home that is putting your
daughter in a chair or taking it to the bed. then raise the pillow until she feels
comfortable.
Family : I just knew about it
Nurse : in addition there are other ways, namely by deep breathing relaxation
techniques.
Family and patient : How to do it nurse ?
Nurse : (while practicing the techniques). First, inhale deeply then hold for 3 seconds.
After that remove it through the mouth. Repeat this technique 3 times. Now
Mrs. Ella and Miss Nindy try to practice what I have explained.
The family and the patient practice the technique of deep breathing together.
Nurse : Yeah that’s right.How do you feel after doing this technique Miss ?
Patient : Alhamdulillah I’m feeling relax nurse.
Nurse : Wah I’m glad to hear that miss
Family : Thank you nurse
Nurse : Nevermind Mam. Hopefully useful and always remembered to do it. Thank
you for your time and cooperation. good afternoon.
Family and patient : Good afternoon.

1 UNDERSTANDING Activity form with make deep breath, slow breath
(hold inspiration to the maximum) exhale slowly,
deeply can also and (Smeltzer & Bare, 2002).
2 AIM 1. Improve pulmonary ventilation
2. increase blood oxygenation
3. Relaxation muscle - muscle the body .
4. Providing a sense of comfort.
5. Divert attention .
6. Reduce pain - pain.
7. Increase efficiency cough
8. Reducing distress
9. Reduce anxiety
3 INDICATION Clients who experience anxiety , stress, depression ,
pain .
4 CONTRAINDICATION *
5 PATIENT PREPARATION 1. Advise patient for BAB and BAK especially first
2. Advise stomach not hungry or glut.
6 SETTING PREPARATION 1. Chair with backrest head and arms
& TOOLS 2. Mattress

7 HOW TO WORK
Orientation
At this stage, the therapies does :
1. Give therapeutic greetings: greetings from the therapist
2. Evaluation / validation: asking about the client's current feelings and
readiness
3. Contract:
1). Explain the purpose of the activity, which is creating comfort and
relaxation of the muscles of the body and mind.
2).    Explain the following rules:
-   The client is ready to follow the instructions from the therapist
-    Duration of 15-30 minutes
-    Each client follows the activities from the beginning to the end
Working Phase :
1. Create a calm environment
2. Asking the patient's readiness for therapy
3. Give the patient a chance to ask / say something (k / p follow up
temporarily)
4. Asking complaints and responding adequately.
5. Try the patient stay relaxed and calm
6. Tar breath through the nose through a count of 1,2,3 .
7. H exhale through the mouth slowly - land while feeling the body in
condition relaxed .
(4 breaths inside )
8. Encourage breathing with a normal rhythm 3 times .
9. Do it breath in followed with shut down eye with tight 4 times
10. Do it breath in followed with menggembingkan cheek 4 times
11. Do it breath in followed with push tongue to the sky - the sky 4 times
12. Do it breath in followed with subdue neck 4 times
13. Do it breath in followed with lift up neck 4 times
14. Do it breath in followed with puff up your chest and hands while related
4 times
15. Do it breath in followed with shrugged as much as 4 times
16. Do it breath in followed with interesting toes direction   axis body
(plantar flexion) 4 times
17. Do it breath in followed with interesting toes away axis body (plantar
extension) 4 times
18. Do it breath in normal 4 times
19. Loosen up and down extremities. .
20. Do it again if the problem reappears

Terminatition Stage :
Evalution
1. After finishing the patient was asked how the results, did the patient feel
more comfortable? Anxiety decreases, pain decreases (adjusts to patient
problems) .
2. Give praise for the success of the actions taken by the patient.

Follow up plan
1. Encourage the client to do it again if the problem arises.
2. Future contracts : agree on activities, time and place (if needed)

9 DOCUMENTATION Document on the nursing record.
