JOINT AFFIDAVIT OF DISINTERESTED PERSONS

WE, ELSA Q. LAVENDIA of legal age, Filipino, with residence and postal
address at Blk 3 Lot 10 Pugo Street, Purok 8, Commonweatlh, Quezon City and MA.
NENA T. AGUILLON of legal age, Filipino, with residence and postal address at 1508
Adarna St. Unit V, Brgy. Commonweatlh, Quezon City do hereby depose and say that:

1. We personally know the deceased JESUS CANILLO LURICO and his wife
PORFERIA BUNDOS LURICO being their neighbors and family friends
since the 1990s;

2. JESUS CANILLO LURICO was born on October 15, 1938 in Buang, Tabaco,
Albay. A copy of deceased’s Birth Certificate is attached herewith and made an
integral part hereof as Annex “A”;

3. We know that said spouses got married on September 25, 1969 and that
PORFERIA BUNDOS LURICO is the first and only legal wife of deceased. A
copy of the spouses Marriage Certificate is attached herewith and made an
integral part hereof as Annex “B”;

4. JESUS CANILLO LURICO died on January 22, 2019 at their residence at

1514 Unit 5, Adarna St. Brgy. Commonwealth, Quezon City. A copy of the
deceased Death Certificate is attached herewith and made an integral part
hereof as Annex “C”;

5. However when his wife PORFERIA BUNDOS LURICO verified his records
at the Social Security System she noted that her husband’s records failed to
reflect her husband’s middle name as reflected in the deceased’s Birth
Certificate1;

6. That for all legal intents and purposes the names JESUS CANILLO LURICO
and JESUS LURICO refer to one and the same person none other than the
deceased JESUS CANILLO LURICO;

7. That we are executing this affidavit to attest to the truth of all the foregoing
facts and for whatever legal purpose this may serve, particularly in
connection with the processing of the death claims and/or benefits filed by
the surviving legal spouse of the late JESUS CANILLO LURICO from the
Social Security System.

IN WITNESS WHEREOF, We have hereunto set our hands and sign on this
