To whom it may concern,

In June 25, 2019 (Tuesday) 11:00 A.M. Philippine Time, Henrick

Yson, my son and a first year ABPS 1A student of the University of the
Cordilleras, has have his scheduled second impaction surgery (dental
surgery). After the surgery the patient will require a long duration of rest
due to the administration of anesthesia and the healing process of the
wound which will take a maximum of 1 week to recover. Due to the process
of recovery, the following dates of June 25, 2019 (Tuesday), June 26, 2019
(Wednesday), June 28, 2019 (Friday) and June 29, 2019 (Saturday) will be
absent during his scheduled classes.

I, Helen D. Yson, the mother of Henrick D. Yson, would appreciate

your consideration and would ask for his exceptions of all missed activities
