AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This AGREEMENT is made and executed at the City of ____________, this day of _____, 20__, by and
between:

UNIVERSAL COLLEGE OF PARAÑAQUE, is a duly registered HIGHER EDUCATIONAL INSTITUTION, with

postal address at 8273 Dr. A. Santos Ave., Parañaque City, hereinafter referred to as the SCHOOL.

-AND-

_____________________________________________, 4 th Year, Graduating Student taking up Bachelor in

(Name)
_____________________________________________ with Student # _____________, hereinafter referred
to as the STUDENT,

WITNESSETH; That

WHEREAS, the SCHOOL, subject to the Terms and Conditions, agrees to issue the Transcript of Records (TOR
for Board Exam) of the STUDENT prior to taking the Licensure Examination;

WHEREAS, the STUDENT agrees to HOLD the Transcript of Records (TOR for Board Exam) if any of the Terms
and Conditions are not met;

NOW THEREFORE, for and in consideration of the foregoing premises, the SCHOOL and STUDENT agree and
stipulated the following:

TERMS AND CONDITIONS

1. REVIEW CENTER. To ensure that the student will pass the Licensure Examination, the STUDENT shall
enrol into a well-known and well-performing Review Center.

2. GRADES. The STUDENT shall get a Passing Grade in ALL of the following:
a. Pre-Board Exam given by the Review Center;
b. Specific Subject Post-test given by the Review Center; and
c. Diagnostic and Pre-board Exams given by the School.

3. REVIEW CLASSES. To supplement the knowledge of the student, the school may provide additional
review sessions. The STUDENT is required to attend the review classes as scheduled by the SCHOOL
and the REVIEW CENTER. Absences are ground for non-release of Transcript of Records (TOR for Board
Exam).

4. RELEASE OF TRANSCRIPT OF RECORDS. In case the STUDENT failed to comply in any of the Terms and
Conditions especially items 1, 2, and 3; the STUDENT waives his/her rights for the release of the
Transcript of Records (TOR for Board Exam).
