Dear Honorable Anthony Reyes: 

Greetings!

Solo Parent constituents from different barangay of Milaor, Caamrines Sur would like to form an
organization for Solo Parent to be able to help all qualified constituents. 

In this regard, we will conduct an initial meeting this coming Wenesday, October 30, 2019 1:00 PM at
_______ with the agenda of Election of Solo Parent Officers and other matters. Upon consultation, we
agreed to coordinate with your good office to respectfully request for benefits availment of solo parent
which is the REPUBLIC ACT NO. 8972 also known as "Solo Parents' Welfare Act of 2000." 

Herewith attached list of the possible Solo parents with signatures subject for verification. Also a copy of the
list of officers which will be needed for the SEC registration. 

Further, we are planning to organize a stable solo parent organization of Milaor, Cam. Sur. This will benefit all
qualified applicants for Solo Parents. We would like to seek assistance and support from your end as the
Mayor of our Town.
