Description of Incident: At 5:21 pm on March 10, 2020, Wu Shaopeng received a report from the Supervisor that the hydraulic oil leaked from the excavator during return trip to work site. At 5:24 Wu Shaopeng arrived at the scene and found that workers were using Spill Containment Kits to deal with the leak. About 5L of hydraulic oil was leaked from observed. At 5:31 Wu Shaopeng reported the incident to the EO of PMU. Mr Wu informed Mr Mathew about the mirror hydraulic spill. At 5:41, the mechanical engineer rushed to the site to repair the hydraulic oil pipe. Mr Wu sent photo of clean-up to PMU’s EO and advised that investigation report will be submit next day. 7. Immediate Measures to prevent Re-occurrence: Immediately arrange a mechanical engineer to repair the damaged hydraulic oil pipe and check the hydraulic system of other mechanical equipment to ensure a secure connection. 8.Immediate/Direct Causes (Substandard Acts and Conditions): Hydraulic tubing connection connector slipped. 9. Underlying/Basic Causes: Hydraulic tubing aging. 10. Loss Potential: MAJOR SERIOUS MINOR Probability of Reoccurrence: HIGH MODERATE LOW 11. Recommendations: N/A
