INTRODUCTION
This chapter contains the history of the Installation or establishment,

mission, vision, objectives, organizational chart, and the facilities.
a. All about the establishment
This edifice and humbly and gratefully dedicated to the people of the City of San
Fernando, Province of La Union.
The lot where the San Fernando City Police Station stand is donated by the City
of San Fernando.
Construction started in 2005 during the 3rd term of City Mayor Mary Jane C.
Ortega and completed in 2008 during the 1 st term of City Mayor Pablo C. Ortega with
the assistance of Congressman Jose de Venecia, Congressman Victor F. Ortega, First
District of La Union, the Provincial Government of La Union thru Provincial Governor
Manuel C. Ortega and the Philippine National Police.
This is model of cooperation among all levels of Government to uplift the quality
of life through the preservation of peace, order and safety for the protection of general
welfare of the community.
Inaugurated this 26th day of March 2009.

DAILY ACCOMPLISHMENT REPORT
Day no. : 1
Date. : January 14, 2020
Type of duty : Regular
Am time in : 7:30 Am time out : 11:30
Pm time in : 1:00 Pm time out : 5:00
At around 7:00 o’clock in the morning, I arrived at the Police Station in Tanqui,
City of San Fernando, La Union for my second assignment of OJT. I together with my
groupmates Followed Ms. Jacqueline Moskito our OJT Coordinator for us to endorsed
and introduce us to the PNP Personnel’s specially We Met police Lieutenant colonel
Percival Rada Pineda the Acting Chief of Police, and also PO3 Yvonne Garcia Who
orient us and introduce the different rooms/section, then after that she divided us into 3
groups. After we group our selves, we proceed to our respected areas were assign.
My first duty is in Admins office together with my companion Ben Joshua Lubrica.
PO3 Yvonne Garcia assign us to measure the BMI of the Officers. We are also given a
task that wen need to help them to the cleanliness of the station. In afternoon at about 1
pm , me and Ben continue our task and also helping the officers Like printing some
documents And also encode. At about 4:30 pm we clean our areas of duty.

Day no. : 2
At 7:30 o’clock in the morning, I arrive at the police station, my first duty is clean
the admin office with my companion Ben Joshua Lubrica. After that, we proceed to our
area of duty then follow the instruction given to us like measuring the BMI of the officers
or printing some documents.
In the afternoon at about 1:10 we continue our duty. PO3 Yvonne Garcia
Told me to file a documents or put into the folder where it belongs then after that I
continue to my firs task. Before 5 pm in afternoon, we proceed to clean our area of duty
to keep the cleanliness of the station as one of our task .

Day no. : 3
At 7:30 am in the morning, I arrive at the police station. I started to clean the floor
by swiping then after that I mop it. At 8 o’clock PO3 Yvonne Garcia ask me a favor to
fetch some water for the CR’s at the 2nd Floor and after that I resume my first task which
is measuring the BMI of the Officers.
At 1:00 pm in a afternoon, NUP Office theresa Gurtiza Ask a favor to deliver a
files from RTOT. At 3 pm we went out to have a snack and we go back at 3:15 pm. At
around 4:10 pm we fetch some water for the CR in the second Floor then after that we
mop the floor and at around 4:55 pm we went Home.

Am time in : 7:30 Am time out :11:50
At 7:30 am I arrive at the police station, me and my buddy Ben Joshua Lubrica
stared to clean the admin, then after that we wait the Officers to arrive to measure their
BMI. At about 10 am PO3 Yvonne Garcia told us to print some documents and put it
into a folder. At 11:45 they told us to have a lunch.
At around 1:05 we go back to our area of duty to continue our task given by the
Officer in charge at the Admin. At 4:30 pm we started to clean the rooms in our area of
duty And at 4:55 we went home.

Date. : January 21,2020
At 7:10 am in the morning, I arrive at the police station, me and my groupmate

Ben Joshua Lubrica , Rowena Velasco, Frances Vencer assign at the desk for 1 week
as weekly rotation. At 8:30 am we already clean CR, the Investigation Office and after
that me and Ben Joshua Lubrica post at the desk to assist the clients who have a
concerns, and also to observe investigators . At around 11:30 in the morning PO3
Manny Toring , brought me with my companions Mr. joshua Opeña and Mr.Ben Josua
Lubrica into barangay madayegdeg pensilvania ave. to demonstrate an actual
investigation of thef. We observe on how to produce a finger print and what are the
information needed.
At 1:30 pm in afternoon, the investigation done then after that we go back to the
station. Before we proceed to our area to post We went to have a lunch then after that
at 2:05 pm we go back to our area’s assigned to duty.

Day no. : 6
At around 7:20 am in the morning, I arrived at the police station, I place my things
in a room Then I started to find the things I need to clean my area of duty. At around
8:30 am in the morning, We joined the NAPCO Police Officer in to their seminar
together with my buddy Ben Joshua Lubrica, Joshua Opeña, John Ezekiel Tubera, the
lecture is about on how to do the Objectives of a report, how to construct, and also
discuss the traffic management. 11:30 am we went out to have a lunch.
At around 1:00 pm in the afternoon, I observe on how to gather information in a
client in order to make a blotter report. NUP Mrs. Theresa Gurtiza teach us on how to
refill the coffee powder in vending machine. At 4:30 pm we started to clean our area of
duty and then at 4:50 after we finish we went home.

Day no. : 7
At around 7:30 am in the morning, I arrive at the police station and quickly I
started to mop the floor. At around 9:00 am in the morning me and my buddy Ben
Joshua Lubrica assist PO2 Orfano on taking a picture of a damage vehicle for filing a
blother report and also the license of the owner.
At 1:00 pm in the afternoon, we started to post on our duty area, and start
observing and also assist some client for their concerns. At 2:30 pm I deliver a
documents came from the desk Officer from the Admin Office. At around 4:15 we
started to clean our area, the investigation office, CR, Desk, and the front desk. At 4:50
pm we went home.

At around 7:30 am in the morning before I started to clean my area of my duty, I

fetch a water for the CR’s, after that I started to swipe leaves at the back of police
station. At around 8:50 am PO3 Opinaldo told me to give a letter to the admin office. At
about 11:40 am we go out of the station to have a lunch.
At around 1:00 pm in a afternoon, me and my group back to our area of duty. At
around 2:30 pm Desk Officer PO3 Opinaldo give me a task to encode a file or report to
change some details on it. At 3:10

At around 7:30 in the morning, I started to clean the area where I am assigned
for my next duty. My group and I assigned for assisting people who applied for police
clearance. At aroud 8:15 am people who applied for police clearance . At 11:30 me and
my buddy ben go out to take have a lunch.
At around 1:00 pm we started to post on our duty on assisting people for the
application of police clearance. At around 4:30 pm me and my group started to clean
our surrounding beffore we pack up our things . At 4:50 pm we went home.

At around 7:30 am in the morning, I arrived at the police station, I started to clean
the area where I assigned for duty. At 8:05 I arranged the table and chairs where we sit
to wait for the applicant for police clearance. At 9:50 am I change the empty bottle of the
water dispenser . At around 11:45 am me and my buddy Ben go out of duty to have a
lunch.
At around 1pm we go back to our sits where we assigned for duty. At around
4:30 I started to clean the area, then at 4:50 we Finally done and ready to go. At 5 we
went home.
