AFFIDAVIT OF SERVICE

I, FRANK C. NATRA, of legal age, Filipino citizen, single, and with

residence and postal address at #1 Trancoville, Baguio City, having been duly
sworn in accordance with law, do hereby depose and state THAT:

1. I am a Secretary at the law office of Atty. Jacquelyn F. Maliaman with

office address at 9F Puso ng Baguio, Upper Session, Baguio City,
Philippines;

2. On January 28, 2020, in my aforementioned capacity, I had personally

served a copy of the Complaint via Accredited Private Courier in
accordance with Sec. 17c, Rule 13 of the 2019 Proposed Amendatory
Rules of the 1997 Rules of Procedure;

3. The Complaint is entitled “Juan dela Cruz vs. Pedro P. Duko for
Collection of Sum of Money with Damages” with Case no. 123456;

4. I had personally served the copy of the above-stated complaint via

LBC Express Courier, plainly addressed to the defendant’s lawyer at
his office at ABC Law Office, #3 Rimando Road, Quezon City,
Philippines, as evidenced by courier’s official receipt no. 123 and with
a tracking no. E123 hereto attached;
