Comes now Petitioner, to this Honorable Court, most respectfully avers the
following:

1. That, petitioner is of legal age, having been born on 28 April, 1977, in

Infanta, Quezon, Filipino Citizen, presently residing at No. 559 Barangay
Ungos, Real, Quezon. With office address at No. 41 Velasco St.,
Pobalacion 1, Infanta, Quezon. The Location sketch of Petitioner’s Office
is hereto attached as Annex “ A” to form an integral part of this petition;

2. That, he has been admitted to the practice of law in the Philippines as

shown by the Certificate of Membership to the Philippine Bar, which
contains the petitioner’s Roll of Attorney Number 52879. Copy of the
Certificate of Membership to the Philippine Bar is hereto attached as
Annex “B” to form an integral part of this petition;
3. That, petitioner is a member of good standing of the Philippine Bar as
shown by the certification issued by the Office of the Bar Confidant of
the Supreme Court dated 11 October 2018, copy of which is hereto
attached as Annex “C” to form and integral part of this petition;

4. That, he is a member of good standing of the Integrated Bar of The

Philippines, National Office dated 12 October 2018, copy of such
certification is hereto attached as Annex “D”. A Copy of the certification
dated 11 October 2018 showing payment of the corresponding lifetime
membership dues is hereto attached as Annex “E” to form an integral
part of this petition;

5. Also attached as Annex “ F” is the Certificate of no derogatory record

/Good Moral Character dated 7 November 2018, issued by the Chief of
Police , Real, Quezon, where herein petitioner is presently residing;

6. That, herein petitioner has paid his Professional Tax for the year 2008 as
reflected in Professional Tax Receipt No. 5615836 V dated 3 January
2018 issued by the Municipal Treasurer of Infant, Quezon, copy of which
is hereto attached as Annex “ G”

7. That petitioner’s Tax Identification Number is (TIN) 902-068-172, copy

of such is hereto attached as Annex “H”,

8. That, petitioner has all the qualifications and none of the

disqualifications for the office of Notary Public;

9. That, herein petitioner has never been charged nor convicted of any
crime involving moral turpitude;

10. That, herein petitioner has no other commission as Notary Public in

any other jurisdiction;

11. That, herein petitioner attached in a separate sheet of paper

specimens of his official signatures, said specimens are hereto marked
as Annex “I”
12. That, herein petitioner likewise attached hereto his three (3) passport
size color photographs each bearing his own signature on the bottom
part thereof, said photographs are attached as Annex “J” of this
petition.

13. That herein petitioner undertakes to submit or report to the Office of

the Clerk of Court copies of all deeds notarized by him.

PRAYER

WHEREFORE, premises considered, Petitioner most respectfully prayed

to this Honorable Court that he be granted renewal of his Notarial
Commission in and for the province of Quezon for the year 2019 and for
the term ending December 31, 2021.

Other reliefs, just and equitable under the premises are likewise
prayed for Quezon City for Infanta, Quezon 8 November 2018
