KNOW ALL MEN BY THESE PRESENTS:


This Memorandum of Agreement is made and entered into by:
SANTO NINO ACADEMY, an educational institution, with principal
office at Gorordo Ave., Lahug, Cebu City, Philippines, represented
herein by its Principal, Sister M. Joellen Ramirez, OSF, hereinafter
referred to as “SNA”;
Celeste Jean L. Abayata of legal age, Filipino and a resident of Brgy.
Sabang, Malitbog, Southern Leyte hereinafter referred to as
“STUDENT TRAINEE”
- and -

MJN Internet Café and Printing Services, an IT business duly
complied under the existing laws of the Republic of the Philippines with
office/business address at San Antonio, Malitbog, Southern Leyte,
Philippines, represented herein by its Owner, Mr. Gabriell Combate
hereinafter referred to as “COMPANY”
WITNESSETH; That –
WHEREAS, the SNA Grade 12 through Work Immersion Program requires
its students to undergo training through this Work Immersion Program in its School
Curriculum to expose the STUDENT TRAINEE to actual work situations in order to
gain practical knowledge and application through experience in the area of
information technology.
WHEREAS, STUDENT TRAINNEE is a Grade 12-Bonifacio student of SNA.
WHEREAS, COMPANY, is engaged in WORK IMMERSION PROGRAM, is
willing to accommodate the request of SNA to allow STUDENT TRAINEE to undergo
on-the-job training in its TV PRODUCTION Department.
WHEREAS, the owner of the COMPANY represented and warranted that he
has the authority from the Board of Directors to represent the COMPANY in this
Agreement.
NOW, THEREFORE and in consideration of the foregoing premises, SNA,
STUDENT TRAINEE, and COMPANY, hereby agree to the following terms and
conditions:

I. DUTIES AND RESPONSIBILITIES

1) SNA shall:
a) Designate a Adviser who shall work closely with the owner of
the COMPANY to supervise the effective implementation of the
Work Immersion Program and activities of the STUDENT
TRAINEE;
b) Provide COMPANY an Endorsement Letter through the
designated Adviser prior to the commencement of the Work
Immersion Program;
c) Through its designated Adviser, be allowed to visit the
COMPANY from time to time to check on the status/condition of
the STUDENT TRAINEE concerned.

2) COMPANY shall:

a) Designate its owner to observe, coordinate with the SNA Work


Immersion Adviser, for the implementation of the Immersion
Program;
b) Have the prerogative to designate areas of assignments
relevant to the field of specialization and work shift of the
STUDENT TRAINEE according to the COMPANY’s
requirements provided that the STUDENT TRAINEE shall not be
designated to or assigned with places and activities,
respectively, that will endanger their lives and/or limbs or pose
an imminent threat thereto;

II. WORK IMMERSION PROGRAM

The COMPANY shall provide on-the-job training to the STUDENT


TRAINEE in the Immersion Program under its jurisdiction.
Details of the Immersion Program shall be discussed with the
STUDENT TRAINEE, the designated SNA Work Immersion Adviser and the
Owner of the COMPANY concerned, and thereafter an Work Immersion
Program Plan for the STUDENT TRAINEE shall be submitted to the SNA
Work Immersion Adviser.

III. DURATION

The duration of the Internship Program shall be from December 11,


2017 to December 22, 2017, or approximately 40 hours on regular working
days.

The STUDENT TRAINEE shall not be required to render overtime work


by the COMPANY. The STUDENT TRAINEE may, however, opt to render
overtime work, provided, with the permission of the owner of the COMPANY.
IV. WORK RELATION

No employer-employee relationship shall exist between the STUDENT-


TRAINEE and the COMPANY for the duration of the Work Immersion
Program.

V. ALLOWANCE/COMPENSATION

The STUDENT TRAINEE is not entitled to any compensation,


allowance or honorarium and the like from the COMPANY. The COMPANY
may, however, grant the same to the STUDENT TRAINEE in accordance with
its existing policies, rules and regulations.

VI. MONITORING

The STUDENT TRAINEE shall strictly observe the COMPANY’s


existing policies, rules and regulations, or memoranda regarding and/or
governing its employees during his/her Work Immersion Program and shall
exercise care and diligence in the tasks assigned to him/her.
The COMPANY shall monitor the performance of the STUDENT
TRAINEE ensure that the policies and procedures for the Work Immersion
Program are followed, and a weekly evaluation report thereof shall be
submitted to the designated SNA Work Immersion Adviser.

VII. INTELLECTUAL PROPERTY

Any intellectual property owned by the parties prior to this Agreement


shall continue to be owned by them. The STUDENT TRAINEE shall not and
cannot use any confidential information or data from the COMPANY to create
intellectual property without the express written approval of the COMPANY.
Any intellectual property independently created by the STUDENT TRANEE
during the Internship Program shall be exclusively owned by the STUDENT
TRAINEE. Should the COMPANY provide financial support for the creation of
such intellectual property, the ownership of the intellectual property shall be
determined by agreement of the parties which shall be embodied in a
separate written agreement. In case the STUDENT TRAINEE jointly creates
intellectual property with an employee of the COMPANY, the intellectual
property shall belong to the STUDENT TRAINEE and the COMPANY jointly
in equal shares, unless the parties agree on a different proportion of
ownership.

VIII. LIABILITY
The COMPANY shall not be liable for any injury or loss, personal or
pecuniary, that the STUDENT TRAINEE may sustain or suffer in the
performance of their duties and functions arising from the conduct of its
employees, and in the course of the Work Immersion Program.

The STUDENT TRAINEE shall be personally responsible for any and


all liabilities for damage to property or injury to third persons arising from
intentional or negligent acts in the performance of his/her duties and functions
while in the course of the Work Immersion Program.

IX. COMPLETION

The COMPANY does not guarantee employment to STUDENT


TRAINEE following the completion of the specified Work Immersion Program
period and reserve the right to terminate the STUDEN TRAINEE during the
Internship Program period according to its standards, policies, rules and
regulations.
Upon completion of the Internship Program, a Certificate of Completion
shall be issued by the COMPANY certifying that the STUDENT TRAINEE has
undergone on-the-job training from December 11, 2017 to December 22,
2017 rendering approximately 40 hours as TRAINEE of the COMPANY.

X. TERMINATION

This Agreement may be terminated by any party hereof at any time


prior to the expiration of this Agreement, for just and valid causes provided for
by law.
IN WITNESS WHEREOF, the parties hereto, through their respective
representatives, have hereunto set their hands this December 5, 2017, Malitbog,
Southern Leyte, Philippines.

Weekly Activity Report

Tech-week Starter Trainee

Here comes the moment to witness one’s learnings into actions. I can still
remember what I felt as I embarked an unfamiliar step in student’s life. The moment
when you are about to use what I had learned in my months of intensive learning on
my chosen track.
I was in mixed emotions but surely, I felt excited. It was the 11 th of December
2017 when I started to have a wider grasp of IT. I had my training in MJN Internet
Café. Though it was a simple and nothing much to brag about, yet, it was quite a
humble and fun experience. I can’t spell out the words since I can’t contain the roller
coaster like of emotions brought by meeting new acquaintances, enhancing my
relations towards other people (using my lambent smile haha), and being able to
learn what professionalism is in its simplest.

In relation to this, there were few activities done in my first week as a trainee.
First, ID package making in which we were able to print ID laces for the first time. I
was able to experience first hand how to edit photos using a software program,
known as Adobe Photoshop. Second, we accepted IT services such as encoding,
printing, scanning, and photocopying documents. Lastly, we accepted services such
as downloading files and took photos for ID printing using DSLR camera.

Though the experiences were simple and could be mastered with less hassle,
the fact that we deal costumers everyday makes it difficult. Composing oneself in
dealing with costumers in different walks of life is difficult. On the good side, I was
able to practice professionalism and public relations in its simplest. In line with this,
nothing beats the good relations and fond memories with my co-trainees. They made
my every day in my first week way much lighter than I expected.

Giving Farewell a New Meaning

Second week at last! Nothing beats the excitement of finishing our respective
trainings due to the spirit of the yuletide season. However, grasping the fact that
everything has its own endings made me realized that time should be spent well with
your newly met acquittances. I will enumerate the stuff that I did during my second
and last week as trainee. First, I was given the task to deliver the proudly made IDs
that we made to San Isidro National High School at Tomas Oppus. I was quite a bit
excited but instead of showing my confident self, I was yelled by the principal in his
office right after he saw the ordered IDs. Embarrassment as it may seem because
that was the first time I was yelled by a mere stranger in front of the educated
people. If only he would act in the way that positioned his rank. Nevertheless, I set
aside those feelings and was able to take all in to myself. So, I went back to MJN
Internet Café and told my manager what happened. After, I sat down but I can’t
control my eyes. Tears went down, and my other co-trainees noticed it. So, I was
relieved in a way that my aching feelings subsided with their comforting words. With
the IDs returned, we somehow managed to fix the problem and moved on.

Printing shirts is one of the highlights on my second week as trainee. The


manager taught us well in his step by step manner of giving instructions and demo.
Moreover, with the activities and the problems that we dealt with, we got a hang with
IT computer servicing and boosted our confidence in having sincere and quality
services. Given the spare time that we had, we devote ourselves in ensuring the
customers to be satisfied with our services by maintaining the workplace clean.

Lastly, we bid farewell to each other since we already had accomplished our
work immersion. Farewell though is a sad concept to grasp but I want to redefine the
term. The best thing about work immersion is I was able to meet new acquaintances
and surely thing to meet again and being able to lend a hand for each other’s
improvement. Farewell, after all, is to welcome one another in a different place and
time.

Work Immersion Highlights

As a trainee in the institution I rendered my service was an opportunity for me


to handle myself better in the field of work. I was able to get along with people, and
how to be an asset in working with a team. It helped me spawn to analyze actual
competencies that make good service to people. Though there were times that I had
told and observed conflicts in the work area, I was reminded on how to manage
feelings well. This I mean by expressing appropriately and effectively. In this case,
me with my co-trainess work together smoothly towards our common goals. It is a
good thing that I keep in mind always on how to handle emotions and impulses.
Embracing the highs and lows of a student trainee is never easy, so I had my way of
motivating myself as to hone more my emphatic side and social adroitness. As it
ended, it means I was expected to be mature enough to handle real work situations. 
 
Listening and oral communication is vital when working with people with
different working pace. This goes hand in hand with my adaptability and creative
responses to any setbacks at work. Though the first week, I lacked initiative,
however, I was able to manage it personally. Boosting my confidence in work toward
the goals of the institution is a way to attain my sense of wanting to develop and was
able to take pride in accomplishments. I was reminded to be grounded and establish
group and interpersonal effectiveness, cooperativeness and teamwork, and skills at
negotiating disagreements. 
 
More so, as a student in a SNA, I should embody the values and teachings of
my school and learnings brought by the work immersion program to break any
grounds that I may step. With its end, I am now excited and prep-up to break another
ground in another higher level.

Good Day! I hope this application letter finds you well.

I’m Celeste Jean L. Abayata, 18 years old and a Senior High School learner from Santo Nino
Academy taking up a Vocational Track ICT Information and Communication Technology. I
am writing this letter to expressly show my intent to be part of your personnel for the pursuit
of learning and develop the skills related to IT servicing which is relevant to my chosen track.
This is in line with the K-12 curriculum mandated by DepEd which required all the 12 th
graders from all schools, regional and national to undergo an on-the job immersion as part of
the learning and developing for study among the learners.
The privilege of working in the institution would be a challenging task. Rest assured, I am
willing to do an extra mile and show my confidence for any assigned tasks to be rightfully
carried out successfully. Being a consecutive honor student, this school year surely is a plus
in dealing tough situations able to handle situations under my work ground and out. Hence, I
