Respectfully Referring to your good office for appropriate inquest proceeding the
living person (Name of Respondent), 31 yrs old, married, police officer and presently
assigned at Pasig Police Station of Eastern Police District and residing at Danlog, San
Jose Camarines Sur (Under Arrest) for violation of Grave Threat in rel. to R.A. 7610
& Grave Threat on complaint of the following to wit;

1. (Name of Complainant) A, 16 yrs old, single, student, female, and

presently residing at 2131 Anacleto St. Sta Cruz, Manila
2. (Name of Complainant B), 26 yrs old, single, tricycle driver, residing at
2101 Rizal Ave. Sta Cruz, Manila
3. (Name of Complainant C), 21 yrs old, single, jobless and residing at
1065 Int. 12 New Antipolo St. Tondo, Manila.

Incident transpired on March 30, 2017 at about 3:00AM along Yuseco St. cor
Rizal Ave. Sta Cruz, Manila.
