Neurology rotation (Kiran Kaur PGY-2):

To bring: ophthalmoscope, reflex hammer, safety pins (use a new one each time), clipboard,
stethoscope, pens, pen light

Teresa RN: she is awesome. Will be super helpful with all of the information you need. Get there at 7AM
and go to neuro conference room. You will get access to the back door on the first day. The lists are in
their office across from Neuro conference room in Teresa’s mailbox – leave 2 sheets for her and take the
rest. Make a group chat with the new residents/students and add Teresa to it. This will help you locate
your team when you are done rounding on your patients and keeping her updated.

Dr. Hughes: Likes blue sheets for new consults. Writes own NEW consult notes. Progress notes by you –
Teresa note neuro consult v3. Rounds together on all patients. No stickers – get facesheet

Dr. Jacoby: No blue sheets. Rounds on follow ups alone. Rounds on new patients together. NEW
consults: Neuro consult note under Dr. Jacoby name – still do progress notes under Teresa’s note. Call
patients by Mr. or Mrs. ____. Do not say past medical history. Do not say + Babinski (say extensor
plantar response). Don’t say CVA. Get 2 stickers. Don’t use + signs for reflex. Does use 5/5 for strength.
Just put # for reflex and for strength. He does not like it to be fractions.

Dr. Babikian: Starts ASAP. Likes blue sheet for NEW consults. Teresa progress note for others. ONE
sticker. Do ROS (at least 3). Don’t put imaging results in your plan. He will cut you off multiple times even
if you’re about to tell him exactly what he’s asking. He will have you repeat things a lot. Gets slightly
distracted and Teresa puts him back on track.

Dr. Schmolk: Super nice. Has her own consult notes. Progress notes are the same. Will start off later in
the day but once she starts she cruises through patients. Teaches and is kind about it. Does not need
facesheet or stickers.

Dr. Wright: Rounds at 1030, use Jacoby/Kahout NEW consult notes. Progress notes are the same. He is
very nice but you need to make sure he is organized – he appreciates the help. Works with notes with
you over the lunch hour.

Dr. Prucelli: Curses a lot

Dr. Adelman/Freedgood: Work half days, alternate days.


Neurologic Examination:

Mental Status: Alert. Awake. Oriented x 1 (to self). Language fluent. Good comprehension. Good
attention. Poor memory.

Cranial Nerves: Pupils equal and reactive to light. Visual fields intact. EOMs full. Right facial droop
improving. Mild right sided ptosis. Hearing intact. Facial strength normal. Tongue midline.

Motor: Drift absent. 4/5 strength in bilateral upper and lower extremities.

Reflexes: 2+ in bilateral upper and lower extremities. Extensor plantar response in right lower extremity.
Normal plantar response left lower extremity.

Sensation: Intact.

Coordination: Normal.

Gait: Unable to assess.
