Assist : Good morning, Doctor Jensen's office. How may I help you?

Patient : Hello, I'd like to make an appointment to see Doctor Jensen, please.

Assist : Have you been in to see Doctor Jensen before?

Patient : Yes, I have. I had a physical last year.

Assist : Fine, what is your name?

Patient : Name.

Assist : Thank you, Ms. Name, let me pull up your file... Okay, I've located your
information. What's the reason for your making an appointment?

Patient : I haven't been feeling very well lately.

Assist : Do you need urgent care?

Patient : No, not necessarily, but I'd like to see the doctor soon.

Assist :  Of course, how about next Monday? There's a slot available at 10 in the
morning.

Patient : I'm afraid I'm working at 10. Is there anything available after three?

Assist : Let me see. Not on Monday, but we have a three o'clock opening next
Wednesday. Would you like to come in then?

Patient : Yes, next Wednesday at three would be great.

Assist :  All right, I'll pencil you in for three o'clock next Wednesday.

Patient : Good afternoon, midwife!

Midwife : Good afternoon, ma’am. Whan can I do for you?

Patient : I want to have my pregnancy examined.

Midwife : Is it your first pregnancy?

Patient : Yes, it’s the first pregnancy. I had two months of pregnancy.

Midwife : Okay. Now, whats your name?

Patient : Name.

Midwife : Do you have any problem with your present pregnancy?

Patient : Yes. I have stomachache, nausea, and a headache. I can’t eat anything.

Midwife : How many days do you feel it?

Patient : I think it is two days ma’am.

Midwife : This is not good for your digestion. You’re pale and sick.

Patient : Really? What is my pregnancy will be okay?

Midwife : Don’t worry! Now i will do physical assesment starting from your general health
check from head to toe.

Patient : Allright, please.

Midwife : Well, it’s done. You may sit here. I will tell you about the result. Now your baby
is fine, but you’re limp because you don’t eat anything. I’m going to give you
some medicine. This is medication to prevent nausea.

Patient : Oh thank you. Can you tell me about what I have to do to be healthy during my
pregnancy?

Midwife : You should eat regularly and eat nutritious food, such as green vegetables and
fruit. You must take a rest. I’m sure you’ll be okay.

Patient : No more what should i do?

Midwife : You can follow pregnancy exercise. Pregnancy exercise is very good to do. It can
train your breathhing and flexing your muscles.

Patient : That’s good idea. Where i could follow the pregnancy exercise?
Midwife : You can follow in class for pregnant women. Pregnant women class every Friday
in here.

Patient : Well, i will follow it.

Midwife : I’m glad to know that. Pregnant women class is on the second floor at nine
o’clock.

Patient : Okay, i’ll be there later.

Midwife : This is your medicine. You should take one tablet one a day.

Patient : I hope i’ll be okay.

Midwife : I’m sure you’ll get well soon.

Patient : Thank you very much.

Midwife : You’re welcome.

Patient : doctor , what shall I do for an antenatal examination? 

Doctor : We'll take your weight and have you measured . We have to take your blood pressure , too. 
P : My blood pressure is always OK. 
D : But this is a special time. 
P : OK. 
D : I'd like to know when your last menstruation was. 
P : It was on Aug . 14,1986. 
D : Are your menstual cycles regular? 
P : Yes , they are , I think . 
D : In that case , your due date will be May 21,1987. 
P : In May . It's good . I mean the weather , of course. 
D : Yes . And if you have bleding , watery discharge , pain in your lower abdomen and legs , please
tell me . 
P : when I am tired , I have some pain in my legs. 
D : If you have heriditary disease . You'd better not hide it from your doctor , for he'll tell you what
to do in order to make your delivery safe. 
P : I don't think I have any. 
D : I suggest that you have an examination of routine blood and urine , liver function and blood
group, take more nourishment and avoid sexual relations.

Patient : Doctor , I really worry about my feet. They are swollen , and my fingers . too . 
Dctor : do you have any dizziness , nausea , vomithing , headache or Blurred vision? 
P : Only some headache. 
D : Well , your blood pressure is 160/95mmHg.. And your eyelids , a little bit swollen , any
bleeding? 
P : No. 
D : Your routine urine exam shows that the urine albumin is positive with 2 plus . Omit salt . We
call your problem as toemia of pregnancy . You've got to be admitted to hospital . 
P :Then I'd like to be admitted tomorrow.
