Patient  : “My name is Mrs ayu, nurse”

Patient  : “I have a problem with my head”

Nurse: have you ever had this problem before?

Patient : yes,l have

Nurse : how long have you been having headache?

Patient: 3 Day

Nurse : do you have any alergiess?

Patient: no ,l don’t

Nurse: did you take any medicine before coming here?

Patient : no,l didn’t

Nurse : what is the name of your insurance ?

Patient : its BPJS

Nurse   : “oh I see, sit down please.”

Patient  : “yeah thank you nurse”

Nurse   : “What is a problem with your head? What is your complaint Mrs? ”

Patient  : “Yeah, my head is spinning and it hurts a lot

Nurse   : “Oh I see, and then Do you want to check your head ?”

Patient  : “My General Practicioner is dr Burhan Siswoyo”

Patient: I rarely eat because I have no appetite because I love I like to eat fast food and
spicy food so sometimes I eat spicy food 3 times a day

Nurse : good morning miss , my name is risma and Iam here to take your body’s
temperature, are you willing,is your name Miss khurin?

Patient : yes, of course i'm alone

Nurse : oke miss, iam gonna take your tempature from your armpit, so can you raise your
right hand for a little so that i can put the termometer on your armpit?

Patient : sure

Nurse : miss, you have to hold it in your armpit gentlely for 5 minutes. After 5 minutes we
can get the result.
Patient : is it okay if im doing this while reading a book

Nurse : of course miss, just dont let the termometer out of your armpit.

Patient : oke

Nurse : excuse me miss, its been 5 minutes, can i have the termometer?

Nurse : oke, we get the result. So your body’s temperature is 37 degree celcius.

Patient : is it normal?

Nurse : yes, its normal miss. since i’ve got your body temperature measure, then the
temperature measurment procedure has been completed and i’ll be back later tonight to
do the same procedure.

Nurs: well,l Will go to nurse station,so have any question Miss?

Patient: no ,l haven’t thanks a lor nurse see you

Pulse and respiration measurement

Nurse :good evening, my name is risma, what is your name miss?

Patient : my name is khurin

Nurse : okay, miss Khurin. I need to measure your pulse rate. Now,so Are you willing?

Patient : sure,nurs

Nurse : were you comfortable with this sitting position miss?

Patient : yes, iam comfortable

Nurs : before that I will explain to Miss about the pulse

check is to find out how many times the arteries expand and contract in one

minute in response to the heartbeat. The number of pulses is generally the same as
the heart rate, because the contraction of the heart causes an increase in blood
pressure and pulse rate in the arteries
Nurse : oke, i will measure your pulse from your right hand. So can you put your right
hand on the table?

Patient : sure,nurs

Nurse : i will start measuring, please be still for just a minute.

Patient : yes,nurs

Nurse : oke miss, i have obtained the result. Your pulse rate is 70, your pulse beat
regulary. And as i calculate your pulse, i also calculate your breathing, and the result is
that your breathing is normal at 14 per minute regulary. So there is no problem with your
pulse or your breathing. Is there any question miss?

Nurse : oke miss, since the procedure had been completed and i had the result, so i excuse
myself. after this the doctor will come to furter examination, till then if you need
