Acknowledgement of Risk and Release of Liability
Every person MUST read and understand this waiver before participating in club activities. This agreement will affect your legal
rights. Please read this waiver before signing.

In consideration of being allowed to participate in any way in Kelowna Badminton Club sports program, related events and
activities, the undersigned acknowledges and agrees that:
1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis
and death, and while particular rules, equipment, and personal discipline may reduce this risk, the potential for serious
injury does exist; and…
2. That “Activities” includes but is not limited to sports, fitness, functions, instruction, use of the facilities, participation in
programs, tournaments and services provided by Kelowna Badminton Club; and…
3. I freely accept and fully assume all responsibility for all risks and possibilities of personal injury, death, property damage or
loss resulting from my participation in the activities involved in this program. I agree to accept these risks and agree to the
terms of this waiver EVEN if Kelowna Badminton Club is found to be negligent or in breach of any duty or care or any
obligation to me in my participation in the Activities involved.
4. I confirm that I have reached the age of majority in the Province of British Columbia.
5. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any
unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to
the attention of the nearest official immediately.
6. In addition to consideration given to Kelowna Badminton Club for my participation in Activities, I and my heirs, next of kin,
executors, administrators and assigns (collectively my “Legal Representatives”) agree
 To waive all claims that I have or may have in the future against Kelowna Badminton Club;
 To release and forever discharge Kelowna Badminton Club from all liability for all personal injury, death, property
damage or loss resulting from participation in Activities due to any cause, including but not limited to negligence
(failure to use such care as a reasonably prudent and careful person would use under similar circumstances), breach
or any duty imposed by law, breach of contract or mistake or error of judgement of Kelowna Badminton Club; and
 To be liable for and to hold harmless and indemnify Kelowna Badminton Club from all actions, proceedings, claims,
damages, costs, demands including court costs and costs of a solicitor and client basis, and liabilities of whatsoever
nature or kind arising out of or in any way connected with my, or my guests’ participation in Activities.
7. I agree that this waiver and all terms contained within are governed by the laws of the Province of British Columbia.
I hereby irrevocably submit to the exclusive jurisdiction of the courts of the Province of British Columbia.
I CONFIRM THAT I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT IN ITS ENTIRETY AND

Membership Information sheet
General information:

Please submit this form & payment to a KBC executive or to our membership drop box in the
lobby. For more information on how to renew membership visit www.kelownabadminton.ca
It is mandatory to sign and submit the waiver form that accompanies the membership
application.

Once registered as a new or returning member you will be issued a shoe tag color-coded for
the current year. We ask that you attach it to one of your shoes to identify you as a paid up
member for the current year.
Section 1. Junior members
You must complete one form for each junior member, but for multiple junior applications
parents or guardians only need to enter their own information once if all applications are
together. You need only pay for the first two juniors in a family, but you will still need to fill in
a form for each junior player.
As junior members can only play when an adult member (or coach) is present, junior members
are not typically provided access cards. But cards may be provided at the discretion of the club
in consultation with the head coach and the parents/guardians.
Section 2. Restricted Access
Restricted access means that play is limited to Fridays and Saturdays 3 PM to 11 PM, Sunday 9
AM to 11 PM. Outside these hours drop in fees apply.

Section 3. Access cards:


New members will receive 1 card free. Extra keychain FOB is available for $10. Lost cards are
subject to a $5 administration charge. Cards should be retained yearly and returned if
membership is not renewed. Your card will be ready for pick up within 1 week of a new
membership application being processed.
KBC Rules and Regulations
Since the club runs on an honorary system, members are expected to ensure that any guests and fellow
members follow these rules, so everyone may enjoy the club for years to come.

1. INDOOR COURT SHOES must ALWAYS be worn in court area. It is essential to carry indoor shoes into the club
and wear different shoes to the club so that we do not track in sand and dirt from outside which will damage the
wooden floors. There are donated shoes in both locker rooms for people to borrow if needed.

2. SHOE TAGS for the current year must be worn by members. This enables members to recognize other paid up
members.

3. GUESTS must come with Member, Sign in & Pay Drop-in Fee BEFORE entering the court area.
 The member who invites the guest must sign them into the guest book provided. If a guest is not signed in
they will not be covered by our insurance.
 If a visitor is not accompanied by a member and you allow them in, you become responsible for their visit.
You must sign the visitor in the guest book and ensure that they pay the drop-in fee. They must leave when
you leave - unless another member is willing to take responsibility for them at that time.

4. PROTECTIVE EYEWEAR: Anyone under the age of 19 MUST wear protective glasses at all times when on court
playing. This is mandatory and all members are expected to enforce this rule.

If you bring in a junior to play, you are responsible to ensure they are wearing protective eyewear. There are
glasses available for juniors to borrow.

5. ENTRY CARDS are NOT to be loaned out to ANYONE. This includes family members. This is to ensure the safety
of members and the club. Members will be warned once, and then risk having their entry card revoked for a
second infraction.

6. VACATE COURTS when others are waiting.


 Singles are not to be played during Club Play times if others are waiting. Playing times are noted on the
bulletin board in the lobby and on our website.
 During busy times please vacate the court after your game so the next foursome can play.
 Juniors are not to be brought to the club to play during Adult Club Play time. During the off-season, this rule
can be relaxed as long as no adults are waiting for a court. The club does allow adult playing privileges to
some juniors, at the executive’s discretion.

7. KITCHEN - Return borrowed cups and clean kitchen after use.


If you borrow a cup from the kitchen for water, you are expected to return it and place it in the dishwasher.
As a courtesy to other members, clean up after yourselves. Please run the dishwasher when it is full.

8. LAST PERSON TO LEAVE - must make sure doors are secure, LIGHTS ARE TURNED OFF, and HEAT IS TURNED
DOWN upstairs. Court lights are on motion sensors and will turn off automatically in a few minutes of inactivity.

9. CELL PHONES - Please turn OFF with the exception of people who are on call, etc.
