Dear Mr. John,

Dear John,

I am writing you to narrate the incident of injury occurred on workplace with one of our
team member “Garry King”. Garry is the most senior and loyal employee to our
company, and serving MacVille from last 30 years. In actuality, he was working at one of
the site to repair espresso machine, and during the work he got a severe back injury and
has recommended rest by the doctor. As you know, he is the most experienced employee
and sometime travels to the customer sites to repair the espresso machines.

Furthermore, while investigations about the incident I found that there are few potential
health & safety risks with the workplace practices which need to be reviewed on the
urgent basis. Garry was working alone on the sites where there should be two of the
workers to lift the heavy machinery. Moreover, we also need to modify our rotating
roster system. I would suggest the following recommendations as a precautionary
measures to avoid the incident of the same nature in future:

 A flexible rotating rosters for the employees with proper comfort breaks
 In warehouse, workbenches and storage of tools should be changed according to
convenient of the workers
 Forklifts should be used to lift the heavy objects
 The mechanic should be provided with a helper for off-site work

I assessed the work place practices by keeping the OHS ACT 1995 in mind.
