TMJ Case Studies

The first case study was about a patient who has a habit of chewing on one side of her

cheek. She is receiving care from both a dentist and a physical therapist. It is noted that

disorders affecting the TMJ frequently cause spinal pain, as this patient is experiencing some

issues with her cervical vertebrae. The PT student discussed the three key muscles involved in

this case. In the video provided by the PT student, the woman discusses that it is important to

assess the jaw symmetry. Just as we do in the clinic, it should be noted if the patient has any

clicking or popping. The treatments recommended in this case study were dry needling of the

muscles involved as well as some stretching of the muscles involved. OMT treatment was also

recommended in this case.

The patient involved in the second case study grinds her teeth while sleeping as well as

chewing ice and her nails when under stress. The pain the patient experiences gets worse as the

day progresses and disappears when she is not under stressful situations. In the case study it

states that the patient is receiving care from the dentist and physical therapist. Treatment

involves strengthening of the muscles, relaxation techniques, and pain management modalities.

The PT student discussed the main muscles effected. Exercises should be performed on these

muscles to improve muscle tone and help to relieve certain trigger points.

This project showed me how important interprofessional collaboration is. Although we

have learned how to properly assess for TMD, it is not part of our job to provide this kind of

therapy. As hygienists, it is important to check all of our patients for any variants from normal
and to refer whenever necessary. Most patients will be seeing a dental professional more

frequently than any other health care worker, so assessment of our patients is crucial.

Thoroughly checking the TMJ for symmetry at each appointment and asking questions about the

patients habits can prevent TMD conditions from becoming extreme. Improving muscle tone

and teaching the patient ways to relieve stress and practice mindfulness can help the patient in

the future. Working alongside a physical therapist is important because they specialize in

providing exercises and stretches to patients to improve their lives by reducing pain.
