immersion experience has a big impact on my outlook in the working field. It gave me
new knowledge and experience, and I hope that as you read this narrative it will give
you inspiration and more passion to your job training and to your future job you want
to take.
My experience was very fun and it was new to me. I and my classmates have been
assigned to different kinds of departments. We had our own tasks and duties that the
head of our departments. We learned how to give services and provide the needs of our
clients. The fulfilment and satisfaction is priceless and I am glad that this training
happened.

BIOGRAPHICAL SHEET
Naomi I. Seregal is born on October 26, 2001, in Naic, Cavite. She is currently 18
years old. Her parents are Jocelyn Imperial Seregal and Rodrigo Nazareno Seregal, she
also has an older sister named Nichole Seregal. She describes her family as a warm,
honest, supportive, kind and generous people who is always there for her all the time.
They all live in the same household at San Roque Hiway Naic, Cavite. She is currently
a Grade 12 Senior High School student at Notre Dame of Trece Martirez. Maintaining
a good record in her report cards is hard, but all hard works will pay off once she
finishes all her studies and pursue her dream career of being a flight attendant who
travels all around the world while learning every culture of each country.
Her hobbies are singing, dancing, cooking, watching dramas, and making several
clothing pieces. She had a difficult time deciding what her dream job is considering all
her hobbies she once considered being a chef, then a fashion designer, but she knew
her passion is for traveling. She thought that cooking and making clothes can just stay
a hobby to her. So many people have helped her continue to pursue her dreams, her
family, her girl friends she had a long time ago and to the friends she had only met this
She always dreamed big, she never lets opportunities come and go, she always
grabs the opportunity because she knows her family has her back and would support
her no matter what. She is motivated to create the best possible future for herself while

I want to express my most profound thankfulness for the people that helped me
satisfy this humble of work; without their steady help and direction, my work
immersion won't be conceivable. For everyone who helped me survived till the end of
my work immersion. I have learned a lot.
To begin with, to our Almighty God, for giving us the quality and direction and for
guiding me throughout my whole work immersion experience.
For the head of my department, whom I learned to adore and whom have taught me
many things, and for being so patient through my whole 80 hours of work immersion
and for who had been with every one of the hardships and the problems we looked as
we do ourwork immersion, thank you.
To our immersion teacher, Mrs. Glenda G. Ambon, for giving us thoughts all
through the immersion. For her unending direction and backing. She is with us all
through every one of the issues that we had experienced; her advices and reactions
supported the composition of this narrative from multiple points of view.
For my parents who are supporting me and for making me feel happy that I made
them proud for having my immersion finished. Furthermore, to our cherished
guardians who readily helped us by methods for monetary help as well as giving us
unrestricted love and support. Their undaunted help of this task was incredibly required
by the researchers and is profoundly valued.
To my friends Enrique, Franzyne, Angela, Mica, Erol, Crisa, and Franziel thank you
for all your love and support..
To myself, for making effort and for making it through the end. It may have been
hard at first but it was all worth it.

CHAPTER I
INTRODUCTION
The Department of Education (DepEd) issues the Guidelines for Work Immersion
as basis for the implementation of work immersion in all Senior High Schools. Part of
Senior high school curriculum is work immersion. As a senior high school student, we were
deployed at an health insurance company, as per order no. 30, 2017, a grade 12 student
must undergo work immersion.
My chosen career path is a flight attendant, but instead of having a work immersion
in a field where I can develop and establish my skills and knowledge about my chosen
career, I was deployed and trained in a health insurance company. Still, with a minimum
time of 80 hours in the work immersion I have gained understanding of how things function
in the work field. My work immersion involves hands- on experience or work simulation in
which I can apply my acquired knowledge.
Work immersion will help develop senior high school students like me. It will help
students to test themselves and applied what we learned in a non-school scenario. We will
be able to develop many skills and values that would help senior high school students as we
transition to work immersion through our desired career path.
1. Appreciate the importance and application of the principles and theories taught in
the school
2. Enhance the technical knowledge and skills of the students
3. That they may enrich their skills in communication and human relations
4. The students may develop good work habits. Attitudes, appreciation and respect for
work.
CHAPTER II
History
The plan to build a school has long been thought of by the two members of the
Board of Trustees; Mr. NiloPenado and Mr. Marvin Hiponia. When the lots where the
school now stands were offered, they made an on site visit and bought the land. At that
time, the city has three high schools, one private in Barangay Cabuco and two public
schools at the city proper. The location of the school looked good as it is visible along
the national road and not so close in the highway.
The name, Notre Dame of TreceMartires was chosen as it is related to Our Lady,
the Blessed Virgin Mary, to which the school shall be put under her protection. Also, it
aligned with all other schools with similar name which are client schools of HP
Digisystems Co., the computer company owned by Mr. Penado and Ms. Hiponia. Also,
the owners believed that the person’s success in life depends more on his character
among others. Thus, even if the school endeavors to achieve academic excellence, it
intends to be knon more as “The school that builds character”.
The school’s corner stone was laid on Sptember 1998 and soon after the main
building was constructed. The school opened in June 1999 with 96 students in from
Nursery to First Year High School. The first principal was Mr. LouniePenado-Espina.
Later, the annex 1 building was added and the front grounds were cemented. In the
years that follow, the school enrollment rose and then maintained at about 300 students.
With the final implementation of of the K-12 Program more rooms and facilities
were added as we anticipate an increase in enrollment in June 2016, the school opened
its doors to 236 students in Grade 11-Senior High School.

CHAPTER 3
ON-THE-JOB TRAINING
Today is the first day of our immersion. I was with my classmates when we
were deployed at PhilHealth. When we arrived at our work place, we were oriented
by former OJT students which were also from our school. I have met new
schoolmates that seemed nice and now will work together with us. We were taught
on how things work and how things should be. Sir Angelo confirmed to us that we
should start by today instead of Thursday. Inside the office, we were told to do the
PhilHealth Member Registration Form (PMRF) that will be handed out to clients.
After making forms, I help out at the Public Assistance and Complaint Desk
(PACD). Since there are no noon breaks at PhilHealth, we took turns whenever we
are going to take a break so that everyone will get to eat. Today was exciting and
fun, I am looking forward for the next days that I will spend my time at my work
place.

thankfully we managed to finish everything.
Today is Friday, we were told by the staffs that our attire can be just a polo shirt and
pants. On this day Mica Ella and I just helped the staffs assist the clients in the Public
Assistance and Complaint Desk (PACD). The two of us mostly do works together
because we are from the both department. She and I made some PhilHealth Id’s and
gave assistance to supervisors and to our clients too. We also did some PhilHealth
Member Registration Form (PMRF) that will be given to the clients.
Employers Forum was held at PhilHealth today, that’s why Sir Jefferson asked me
to help him with the certificates that need to be handed out after the conference. As I
finished preparing the certificates, Ma’am Rishley gave us a huge bundle of Return To
Hospital (RTH) that came. Mica Ella and I helped each other to finish duties on time.
She taught the both of us how to record Return to Hospital (RTH), she asked us to
input the transmittal numbers as well as the dates included. After putting the
transmittal numbers we arranged the Return to Hospital (RTH) in the claims/benefits
drawer. The Return to Hospital (RTH) were separated through their hospital names.
This day was the busiest yet the most fun one.
Another day for my work immersion. For this day we were asked to prepare the
Certificate of Attendance for the orientation that will be held tomorrow. Ma’am Edelyn
asked Mica Ella and I to prepare some PhilHealth goods that will be handed out at
sixteen towns they will visit. And since we have more time left, we printed out and
arranged some PhilHealth Member Registration Form (PMRF) that is needed till the
end of the day.
Today we released some Return to Hospital (RTH) and asked Franzyne and
Crisamae from the membership department to help us since the both of them does not
have tasks to do from their department. We printed PhilHealth Member Registration
Form (PMRF) and answered questions of some clients regarding health insurance
through phone calls, but whenever we don’t know how to respond we always ask and
seek for help from the staffs. After each phone call they give us information and
familiarity of what we should do the next time another client calls asking for the same
situation, so that next time if another client calls we are aware of what we should say
or do.
For day 7 of work immersion Return to Hospital (RTH) was released today too. I
was helped by the other OJT student from other school which was also in the claims
department. She also helped me return the papers in its own respective folders in the
claims/benefits drawer. It was finished fast and easily so we printed some PhilHealth
Member Registration Form (PMRF) needed for the day.
Clients have asked us to release some Return to Hospital (RTH), many clients
were there and most of them were asking for Return to Hospital (RTH) papers, so
today was a very busy and tiring day. After finishing the arrangement of Return to
Hospital (RTH) papers, Ma’am Rishley asked for my help to organize and fix the lists
of patients for the month of July and August according to their names and the
following dates included in the master list. After sorting out the list, I immediately
returned the pile of papers into their respective boxes and drawers. Then I run some
important errands for some supervisors all around the office.
On this day, The four of us, were asked to put the id’s into their specific
Membership Data Record (MDR), this is done whenever id’s of clients and their
Membership Data Record (MDR) are not together in the same stack. After we are done
with the arrangements, we are now not that busy and doing anything much. We just did
what we did as any other days, we were asked to print and prepare PhilHealth Member
Registration Form (PMRF) and then Mica Ella and I assisted some clients who was
there for Return to Hospital (RTH) because it is for our department, the claims.
Day 10- September 4, 2019
Today is our last day of work immersion we were not doing much in my department
(claims) because some Return to Hospital (RTH) was not released, that’s why we
helped at the Public Assistance and Complaint Desk (PACD). We also helped at the
administration department where my fellow classmate Enrique was assigned, so we
asked for his assistance. We were asked to arrange the survey forms and input the
results in the tally sheets for the month of August and September. It was easy so we
finished right on time, and Ma’am Rowena thanked us for the help we did. we were
late on the first day and we still need to complete the full 80 hours so our working time
got extended. To sum up, we were at the office to complete the one hour late.

should be more dedicated and focused in doing work and do everything heartily with
all I can.
It is very pleasing to know that you accomplished something and it is also very
warm to know that my supervisors enjoyed my company while working with them. It
encouraged me to be more determined in anything that I can do as a student today and
as a professional someday.
This whole work immersion experience was indeed very helpful to students like me
who was afraid of working at first, but now that I have went through a job experience, I
am able to see the sight of what it feels to be on a real working environment.
