I, ERVIN PAUL ENEMENZO, of legal age, single, Filipino and a
resident of 42-2 Bonifacio Extension, Davao City, under oath depose
and say:

1. I am a duly registered taxpayer of the Bureau of Internal

Revenue with Taxpayer Identification No. 436-946-659-0000;

2. I am applying for Additional Exemptions as provided for by our

present tax laws and revenue regulations, as Qualified
Dependent Children, my daughter PRETTIEZEL MATA
ENEMENZO, born on December 12, 2014 at Davao City,
Philippines;

3. That I have provided and continuing to provide PRETTIEZEL

MATA ENEMENZO with SUPPORT as defined by our laws, in
compliance with my parental obligation as her father;

4. That I am executing this affidavit to the truth of the facts and

for such other legal purposes this affidavit may serve.
