International Health

Weekly report on COVID 19

This form is to assist International Health to better understand, how your Health Facilities are coping
with COVID 19, and the amount of strain it is putting them under. It will enable me to understand
their needs as time goes on, and whether there are other ways in which I could support them.

It is a short form and hopefully it will only take a few minutes each week to complete. Where you
have more than one COVID 19 project for your Health Facilities, then I would request that you
complete a separate report for each one.
