forward to Level III, complete and submit this form within ten (10) business days of receipt of notification
of the Level II decision.

Return this form to the Director of Student Services, 302 2 nd St SE, Puyallup, WA 98372. This form will be
submitted within five (5) business days to the OSPI Homeless Education Coordinator with all
documentation from earlier appeals.
