nstructions:
)lease completely fill out this form, Soldiers need to discuss the school with Squad Leader (when applicable) and the Platoon Sergeant.
Jse ATRRS course catalog ([Link] to find scl'rool, school dates and course pre-requisites. lf the school falls on a IDT
rrreekend the commander must approve and sign the request form, The Squad Leader (when applicable) and Platoon Sergeant must
rpprove and sign the document as well, Please remember we want to send the Soldier to school as close to Oregon as possible and use
\G schools as much as possible. We request the school date must be 60 days out or greater. Soidier must meet HTMT, have a passing
\PFT, and in most case have at least 12 months left on their contract. lf the school will re-class the Soldier (MOSO) then the Platoon
Sergeant must discuss MOSQ vacancies with the RNCO for UMR management.
