1. Introduction & Need of Such Protocol.

The Malayali Diaspora world vide is apprehensive about the pandemic


Covid-19. Many Keralities abroad have succumbed to life due to this
pandemic. Many people who have gone to foreign countries on visiting visa
and student visa are stranded there due to the cancellation of all International
Flights to India.
Elaborate guidelines have been issued by Govt. of Kerala regarding the return
of Keralities now stranded abroad due to Covid-19. Online registration on
NORKA website has already been started. The priorities have been enlisted
by the Govt. of Kerala in the following order.
a. Those with time lapsed visiting visa
b. Elderly people
c. Pregnant ladies
d. Children
e. Patients
f. Those with time lapsed visa
g. Students who have completed the education.
h. Persons released from jail
i. Others
Covid test has been made mandatory to all those who wants to return to
Kerala. Journey will be allowed only to those who have tested negative.

A proper protocol is utmost important for the repatriates, the official stake
holders, family members and the general public alike. There are previous
instances where the health worker at Airport was infected with the Covid virus.
A proper protocol has to be there for Pre- departure phase , at the Airport and

post arrival. The most important among these , is the proper management at
Airport . It includes proper screening of travellers , their segregation, data
collection, awareness creation and transportation plan.

2. Facts and figures

Around 3-5.5 lakhs of Non Resident Keralites are expected to return to Kerala
within 30 days , once the travel to Kerala is re-established. Calicut
International Airport expects around 1-2 lakhs NRKs. Out of this around
40000-50000 are expected to be Malappuram natives. Among these 1000 to
1500 may need hospitalisation and 4000- 6000 may have to be
accommodated in Corona Care Centres. Rest can be sent to home isolation
those asymptomatic in thermal screening .

3. NORKA Registration

All the returnees travelling to Kerala have to register with NORKA roots.
Details like address in Kerala, district and local body, Covid test details , and
facilities at home for quarantine are collected at the time of registration. These
details may be shared with concerned district and airport districts. Clustering of
districts may be done for airline booking so that persons from Northern Kerala
return to Calicut, middle Kerala to Cochin and Southern Kerala to
Thriruvananthapuram. This will make transportation planning earlier.

Modification suggested in Present NORKA Registration :-


1.The present and previous symptoms , transportation to home , pre-
existing diseases have to be collected on the NORKA registration.
2. Upon booking the tickets the flight details and travel date have to be
updated. After check-in the seat number also have to be updated.

4. Covid- 19 Real Time Tracking System

It’s an application that help to collect passenger’s data from Calicut


international airport. Real Time Information System has been
implemented by District Administration, along with Department of Health
Services and National Health Mission. The software has been developed
for Covid-19 surveillance, at Calicut International Airport . This will help
to track passenger’s arrival information.Thisdatawillbemonitored on time
at various locations to track the passengers those are affected by
Covid-19 symptoms. Also track their time of arrival in the system to
analyse mandatory Home Quarantine period. The system consists of an
advanced custom report generation programwhichwillhelptofindCo-
passengersifanycaseis found positive,andallows to generate report in
Panchayat wise or district wise based on a flight/date/time period to take
immediate action by the healthdepartment.

The system designed with real-time data tracking module which will run

at passenger arrival areas and a verification module for verifying data is

entered strictly before the passenger leave Covid-19 special surveillance

area. The system is connected to an integrated SMS communication

system which will allow to send SMS alerts instantly to the passenger in

case any Covid-19 related matters found.

The Software is designed using the platform of SAT Software

anditsreleased/implementedasper the protocol of Government ITpolicies.

6. Step by Step activities :

A. Pre Arrival
Just after the arrival at airport the passengers with severe symptoms shall be
segregated first. Data collected on NORKA registration may be shared with
the Airline ground handling staff, so that these passengers may be segregated
by the Airline. The details from NORKA registration may be made available at
least 3 days before arrival. The passenger may be subjected to thermal testing
before entering the transit lounge. Those with severe symptoms shall be
segregated.

B. Segregation of Symptomatic Passengers


Those passengers with severe symptoms will be taken directly to the hospital,
through the tarmac area. Their baggage collection and emigration works will
be facilitated by the airline staff. 108 ambulance will be utilised for the
transportation to the designated hospitals. The segregation and transportation
will be responsibility of APHO with the help of State Health Department.

C. Short Awareness Class to Passengers


After thermal testing other passengers having mild symptoms and no
symptoms will be segregated and taken to the transit area in groups. For
thermal testing and awareness sessions , the positions will be marked to
ensure social distancing. Proper awareness about home quarantine and social
distancing will be imparted during awareness . The IEC materials will also be
handed over. The awareness session will be conducted by the Community
Medicine Department of Manjeri Medical College . The marking at the queue
area and the transit area, other arrangements to ensure social distancing and

arrangements for awareness session will be the responsibility of the Airport
Authority.

D. Data Collection
After the awareness session, the passengers will be taken to the area
before immigration , where the details will be collected online on the Covid
tracker . The data obtained from the NORKA roots and the passengers details
(Flight No. & Seat No) will be integrated in advance with the Covid tracker. If it
is not feasible to collect the flight number and seat number by any method
prior to the arrival, the same may be collected at the arrival lounge. The QR
code will be displayed at the transit areas and arrival lounge and the
passenger can update the details. After the date collection the segregation of
the passenger will be done as follows:
I. Those having mild symptoms
II. Asymptomatic passengers
1. Those From other states
2. Those From other districts
3. Malappuram natives who have to be quarantined at CCC
4. Malappuram natives who have to be quarantined at home
(I) Passengers with Mild Symptoms
All the persons having mild symptoms will be compulsorily
accommodated at CCCs / CFLTC ( Covid First Line treatment Centres ) as
per Health Department protocol. At these CCCs , separate room with toilet
facilities will be ensured. Such CCCs have already been identified and will be
under the direct control of District Administration. The allotment to these CCCs
will be done at the Airport depending upon the vacancies. The real time
vacancy position will be displayed in the Covid-19 tracker. The allotment
counter will be manned by Revenue Department with the help of volunteers .
The transportation from Airport to the CCCs will be arranged by RTO. Buses or
taxies may be arranged as per the requirement.

(II) Asymptomatic Passangers
a.Passengers from other State.

The list of passengers , if any, from other state will be obtained prior to arrival.
Their travel from airport to the state border will be undertaken by the
Malappuram District Administration through RTO. At the state border, they will
have to be received by the concerned state administration. The details of such
passengers will be uploaded in the Covid-19 tracker including the vehicle
particulars.
b. Passengers from other Districts.
Transportation of other District passengers will be in 3 ways.

1. Own Transportation
The asymptomatic passengers from other district will be allowed to use own
transportation if available. Only one passenger / one family will be allowed in
one vehicle. In the pick-up vehicle, other than the passengers only driver will
be allowed. The passenger and driver will have to wear fresh masks
compulsorily. The masks will be made available at the exit counters on
payment basis by Kudumbrasree. The registration number , driver's name and
contact number will be captured in the Covid tracker and the genuineness has
to be ensured by the police at the exit counter.

2. Passengers who take pre-paid taxi


Around 200 pre-paid taxi vehicles are available at Calicut Airport , all fitted with
GPS . Those passengers who want to avail pre-paid taxi facilities may contact
the pre-paid taxi counter at the exit. The police deployed at the exit counter
has to ensure that the passengers who has taken the pre-paid taxi , have
booked then up to their home destination. No halt in between will be allowed.
Only one passenger / one family will be allowed in a vehicle. The passengers
and driver have to wear fresh masks. The fresh mask and sanitisers for the

pre-paid taxi drivers may be made available by the district administration free
of cost. Brake the Chain facility to be installed near the exit counters. Rest
room for the pre-paid taxi drivers may be arranged at Hajj House. After each
trip, the taxi vehicle has to be disinfected and certified. Only after that they will
be allowed to take the next trip.

3. Transportation arranged by the District Administration

Only passengers from the designated northern districts may be allowed at


Calicut Airport. Separate counters will be setup at the data entry area for each
district. The passengers for each district will be transported to a single transit
location, where they have to be received by the corresponding district team.
The transport will be arranged by RTO either in buses or taxies as per the
requirement.

C. Transportation Schemes for Malappuram Natives

The Malappuram natives have to be transported either to CCC or either


houses.
(i) Transportation to CCC
The transportation to CCC will be arranged by district administration
through RTO and Revenue department. They will be transported either in
buses or taxies as per the requirement. The local bodies may also be engaged
in transportation to CCCs, but the allotment will be done at the counter in data
entry area. All the vehicles , which are transporting the passengers will have to
register in the Covid tracker.
(ii) Own transportation / Pre-paid Taxi to houses
The strategy for other district passengers will be applicable here
too.

7. Affidavit

The affidavit has to be submitted by all the passengers who opt for home
quarantine . The facilities for online submission of affidavit may be provided in
NORKA registration. If it is not feasible , QR codes will be placed at the
transmit area and arrival lounges and they can submit the affidavit online with
the Covid-19 tracker. The police at the exit counter has to verify and ensure
this before the passenger leaves the airport.

9. Manpower Arrangements

A. Govt. Officials ( Airport / Police / Revenue / Health )

Posting order of all state Govt staff on duty at Airport will be issued by
District Administration in advance. The airport authority will issue entry pass in
advance to all staff. The staff have to carry photo ID card at the duty time.

B. Volunteers
All the volunteers who are functioning at airport will have to register in
Sannadham portal . Separate registration in Covid tracker will also be
necessary. They will be posted in advance by the Dist. Collector. Accordingly
prior entry passes will be issued by the Airport Authority. All the volunteers
have to carry photo ID card at duty time.

Annexure- 1
Covid Care Centers- Guidelines

TheworldHealthOrganization(WHO)hasdeclaredCovid-19epidemicaffectingmor
e than 199 countries as a pandemic. Due to inflow of persons from affected
countries, Kerala has strengthened the surveillance and control measures
against the disease. A Covid Care Centre (CCC)
isafacilitymeantforquarantiningpersonswhohaverecently arrived in Kerala from
other countries and other states of India. The Objective of a Covid Care
Centre is to enable successful isolation and management of asymptomatic
case.Thes earecentres where the persons reaching Kerala can be
quarantined to contain the spread and manage the people during the period of
incubation.

The Covid centres shall have the following,


1. Independent single rooms with attached toilets
2. Proper electricity, water and internet connectivity
3. Proper food and drinking water facility
4. Security to ensure safety of isolated people

Standard Operating Procedure for CCC

A CCC shall operate under the following standard operating Procedure


1. The district administration shall identify and manage the infrastructure
with the assistance of the LSG. Number of rooms can be flexible as
district administration decides. more rooms can be attached if the
number of cases increases.
2. Identification should be in the order of priority starting with hostels room
a n d
withsingleroomwithbathrooms,thenthehotelroomsandthendormitoriescan
be identified if casearises.
3. The LSG should ensure basic needs such as food, water sanitation,
i n t e r n e t
connectivityandwastemanagement.thelocalwardmembershouldbeinvolv
e d
intheworkingofthecentre,astheLSGisexpectedtoprovidevitalservicessuch
as water, food and cleaning services.
4. ItshouldbeunderthedirectcontrolandobservationoftheHealthInspector(HI)
of the area. If there is no HI, The DMO shall identify a person incharge.

5. There should be at least two Health Care Volunteers (HCV) on duty at
any time per shift
6. There should be a register on the front desk to record all details of the
persons admitted in the CCC.
7. AT the time of admission, there should be a symptom checklist duly
filled by HCV. Daily symptom Checklist duly filled by HCV. Daily
symptom check of inmates should be done by HCVs.
8. If a person becomes symptomatic, he/she shall immediately be referred
to the nearest designated isolation facility hospital in designated
ambulance.
9. Persons with fever, cough, sore throat, dyspnea should not be admitted
in the CCC. all inmates should be sensitized about personal hygiene,
environment hygiene and physical distancing.
10.There should be signage on personal hygiene (cough, sneeze and hand
hygiene) environment hygiene physical distancing and clinical features
of the disease displayed near the front desk.
11.The HCVs should maintain a list of medicines that persons with other co
morbidities admitted in CC are routinely taking. The HCVs must ensure
supply of these medicine in concurrence with the nearby Health care
Institution and LSGD.The demand list must be provided to the
LSGevery3daysanditshould be procured and supplied by the LSG if the
nearest Health Care Institution adequate stock.
12.A doctor of the nearby public health care Institution (PHC/CHC/FHC)
should attend the call, if an inmate becomes symptomatic.
13.The junior Health inspector (JHI) of the area must visit and report on the
CCC to the concerned Health Care Institution on a daily Basis.
14.All the staff members and visitors of a CCC shall practice standard and
transmission-based precautions. if a staff member develops a symptom,
they should self -isolate and report to the medical officer incharge.
15.If a CCC can be converted into a Healthcare Institution later, standard
operating of COVID first line treatment center will apply.

Covid First Line Treatment Centre

The world Health Organization (WHO) has declared COVID - 19 epidemic


affecting more than 199 countries as a pandemic. Due to inflow of persons
from affected countries, Kerala has strengthened the surveillance and control
measures against the disease. since a greater number of primary contacts are
turning positive, and a greater number of patients are being admitted, some
centers need to be identified as Covid first line Treatment centers. The district
administration should decide if the centers can be designated as Covid first
line treatment center based on the following specifications.

The center identified as Covid Health care center should treat all mild and
moderate symptomatic persons under surveillance and should be utilized for
treating positive case, when need a rises.They are the primary level health
care centers for providing care to less serious cases and referral of serious
cases to the Covid hospitals to avoid crowding directly in the Covid hospital
and wastage of resources. The district administration can identify as many
centers for this purpose in line with protocols for isolation.

Services Delivered Through CFLTC

1. Service of health care professionals, appointed by District Administration


2. Logistics, necessary drugs and consumables.
3. Treatment for persons with mild symptoms or category A, as per the
prescribed guidelines.
4. Training for Staff as per guidelines.
5. Observation Area, Treatment Area and Bed strength of at least10-25.

Other Activities

1. There should be an OP register, inpatient register with symptom


checklist register for OP and IPcases.
2. Adequate signage should be displayed in the waiting area, treatment
areas and patient area.
3. A daily report should be sent to district control room.
4. ThecentershouldconductOPfrom8AMtill6PMinrotation.ensurewardduty
and night duty also.
5. There should be a triage area, in front of the OPD, to conduct screening
based on a symptom checklist.
6. Emergency reporting should be done if any person admitted is found
missing from the hospital.
7. Should have provision for biomedical waste management and general
waste management.

Guidelines On Home Quarantine

· Hospitalisationmaybedeferredforpeoplepresentingwithmildillnes
s.Thiswill help in reducing the burden on the health care system.
(except in situations where rapid deterioration in health is
suspected)

· During the period of home care, a communication link needs


to be established with public health personnel.

· Once it has been finalized that the individual will be moving to


his home for quarantine, the below given recommendations should
be followed.

Eligibility of Home for Quarantine:

· Presence of an individual room for quarantine

· Absence of parents/ persons above the age of 60 years or vulnerable


category.

· Feasibilitytoshiftsuchpersonstoarelatives’houseuntiltheperiodofquarantine
of the Covid suspected person

· Access to food without leaving the respective room or home

Recommendations

· HCW will be assigned to inspect the home and approve the


same for quarantine. Other household members should be
informed about the precautions to be taken while receiving the
individual.

· The assigned room should be made ready before the arrival of


the individual. Door of the room should be kept open before
arrival.

· Other house hold member’s should be informed to maintain a
safe distance.
Preferably only one person should be there while receiving the individual. On
arrival, individual’s shoes should be removed outside the home premises.
Shoes should be packed in a separate cover by the individual and placed
separately. Care should be taken by other house hold members, as to not
touch this cover. He/she should enter home only after proper hand hygiene.

· After removing shoes, feet should also be washed before entering.

· U p o n e n t e r i n g h o m e , h e /
sheshouldgostraightintotheirroomswithoutstopping anywhere.
While moving to their rooms, care should be taken so as not to
touch anywhere (eg; stair railings, sofaetc)

· Individual should stay in a well-ventilated room, with an attached


bathroom.

· Movement within the house should be limited as far as


possible. He/she should try to stay in his/her room as far as
possible so as to prevent transmission among other family
members. Also, shared spaces in the house (living room, kitchen
etc should be well ventilated, ie; windows should be kept open).

· Otherhouseholdmembersshouldstayinseparateroom(s).Ifthisisn
otpossible, a distance of at least 1metre should be maintained
strictly at all times. (eg: sleep in a separate bed)

· One healthy member should be assigned the role of caregiver


for the entire duration.

· Visitors should not be allowed, strictly.

· Care giver should perform hand hygiene after contact with the
patient and their immediate environment. (Hand hygiene should
be performed before and after preparing food, before eating, after
using the toilet)

· Clean towels should be used for drying hands and they


should be replaced frequently.

· Thepatient’sclothes,bedlinen,andbathandhandtowelsneedstobecleaned

regularly using regular laundry soap and water or machine wash at 60–90
°C(140– 194 °F) with common household detergent, and dry thoroughly.
Place all contaminated linen in to a laundry bag or a bin with alid.All contents
of the laundry bag or bin needs to be directly emptied in to a
washingmachine or a bucket with soap and water. Do not shake any soiled
laundry and avoid contaminated materials meeting the skin and clothes of
the caregiver.

· In the presence of mild symptoms, use of mask should be


initiated to contain respiratory secretions and should be changed
daily. Proper precautions on how to use the mask should be
taught to the patient and other household members.

· In the event of any surface being contaminated with


respiratory secretion, the same should be cleaned with soap or
detergent and disinfected with 0.5% bleach solution.

· Dedicatedbedlinenandutensilsshouldbemadeavailableforthepat
ient.These items should be cleaned with soap and water by the
patient itself if possible.

· No it ems being used by the patients should be shared by other house


hold members.

· Frequently touched areas in the room like bed frames,


furniture etc should be cleaned daily.

· Bathroom should be cleaned on a daily basis by the patient themselves

· Masks used should be discarded into a closed bin.

· The waste generated within the room should be placed


outside the room in a closed basket at a fixed time daily. The same
should be disinfected by spraying a 0.5% sodium hypochlorite
solution which should further be handled only by the assigned
caregiver.
· The caregiver may use gloves while cleaning, handling the
waste bin etc.

· Avoid exposure to any contaminated material from the patients
