 Attach a copy of the current rabies certificate.
 Attach proof of microchip, unless previously verified.
 If applying for an altered pet license, proof of surgery certificate must be attached, unless previously verified.
 If applying for a senior citizen license, please attach a copy of identification showing date of birth, unless
previously verified.
