1. Interdisciplinary team
The consolidation of background information and assembling of an interdisciplinary team (different viewpoints must be
represented).
2. Problem description
Afterward the root cause analysis will be carried out. In doing this, one keeps asking "Why?" until the actual cause of the
problem has been found. Each time an answer is given to the question "Why?", the answer is probed again with a renewed
asking of "Why?". This way you continue delving deeper until you have reached the actual cause of the problem.
4. Corrective actions
Once you have found the cause, the aim is to consequently figure out suitable corrective actions and to implement them. A
corresponding action plan, with responsibilities and deadlines, supports the implementation.
Immediately after implementation, the result of the action must be checked. Does the problem still exist or has it occurred
again? Were the measures effective? If this involves a recurring measure, such as carrying out of periodic maintenance work,
for example, then regularly repeated checks also make sense.
6. Preventive measures
It must be ascertained as to whether the cause could also lead to problems in other places – i.e. in similar situations,
products, projects or the like – and whether corresponding preventive measures must be defined and implemented.
