Organization Roster and GPA Release Form

All organizations must submit the following form!


I hereby give the Office of Student Activities and/or the Dean of Students’ Office the right to access my current semester and/or
cumulative GPA (Grade Point Average). This entitles these offices to calculate a total organizational GPA.
In addition, this form serves as an official roster of the organization. Official rosters will be used by the Office of Student Activities to
add membership in student organizations to student records.
Please include all members, including officers.
