Sub: - AMC of two nos. OTIS make passenger Elevators installed at “ . Sudha
Hospital,” Kota.

Dear Sir,

Please find enclosed here with herewith our Annual Maintenance Contract form in
duplicate stipulating terms & conditions for the comprehensive maintenance of the
above-subjected elevators. This contract is valid for the period of one year with payment
details as under: -
For Two Speed Elevators: Rs. 20,500/- (Twenty Thousand Five Hundred only) per
elevator, i.e. for 03 nos. our charges are Rs. 61,500/- Sixty One Thousand Five
Hundred only). For Single speed elevator: Rs. 14,000/- (Fourteen Thousand only), i.e.
for 01 no. our charges are Rs. 14,000/- (Fourteen Thousand only). Hence total charges
for 04 nos. elevators is Rs. 75,500/- (Rs. Seventy Five Thousand Five Hundred Only)
for one year. Service Tax @ 12.36% will be in addition to the above charges.
