UNDERGROUND INSPECTIONS
PHYSICAL CONDITION RATING INSPECTIONS

OBJECTIVE

To provide a system whereby planned inspections, follow - up , maintenance, pre - use inspections
and the “5 steps to Safety” program are all carried out to a high standard. Analysis must be
established of repetitive sub - standard condition for immediate rectification by Senior
Management.

MINING

1. Shift Supervisors must rate each contractor under his control once per month, submit his
completed rating forms to the Mine Overseer and Section Manager for their comments and
signature. Mine Overseer to check accuracy of ratings and take remedial action on deviations.

2. Mine Overseer will rate one contractor in each of his Shift Supervisors sections once per
month and submit completed forms to the Section Manager for signature. Also rate all sub -
standards and poor performing contracts monthly.

3. The Section Manager will over inspect one contractor’s rating in each of his Mine Overseer’s
section per month and any contract with a very poor safety or production record.

4. The Business Area Manager will over inspect any rating once per quarter.

5. All rating scores to be recorded on a master file for the yearly score. To be co - ordinate by
Safety Officers.

6. All ratings done by the Safety Department will be submitted to the Mine Overseer for the
Section and Business Area Managers comments and signature.

ENGINEERING

1. The Foreman must conduct a physical condition rating of his section per month or weekly
as per the Legal requirements.

2. The Engineer must over inspect one physical condition rating per Senior Foreman /
Foreman per month.

3. Resident Engineer will over inspect any rating once per quarter.

DAILY

Shift Supervisors to complete field books. Once these are full they are to be handed in to the
Safety Officer.

SAFETY OFFICER

Once the field books have been received they must be compared to rating form submitted.
Any deviations to be high lighted and reported to the Mine Overseer for investigation.

UNDERGROUND VISITS

During each underground visit from Senior Management, Shift Supervisor will record all
items discussed.

When arriving on surface the Shift Supervisor will complete a UG 41 underground visit report.
This will be checked by the Mine Overseer - signed by the Mine Overseer and Shift Supervisor
and submitted to the Manager.
