The Datex-Ohmeda S/5 Arrhythmia Workstation is intended for centralized monitoring and
analyzing arrhythmic ECG data automatically, and measuring and trending ST changes for
telemetry patients and for patients connected to Datex-Ohmeda S/5 monitors.

Introduction

1 INTRODUCTION
The arrhythmia monitoring system consists of the ECG monitors, Arrhythmia
Workstation, and optional telemetry system. This manual consists of following
parts:
- system description
- installation of the Arrhythmia Workstation
- installation of the telemetry system
- detailed information on separate products.
For technical information of the monitors, refer to the manuals of the monitor
used.

Symbols and abbreviations

Symbols
Attention. Consult accompanying documents. In this device the
symbol means that the device must be supplied from an isolation
(separating) transformer when placed in the patient environment.
This symbol is located on the telemetry transmitters. It symbolizes the
Nurse Call button. When Nurse Call is pressed, respective message is
displayed on the ARRWS screen.

Indicates a type CF applied part according to the degree of protection


against electric shock. Indicates also protection against defibrillation.

Safety precautions

CAUTION A CAUTION IDICATES A SITUATION IN WHICH THE UNIT OR DEVICES


CONNECTED TO IT MAY BE DAMAGED.
Operation of the telemetry system is based on radio transmission.
Electromagnetic emission from other radio transmitters, or electronic equipment
emitting noise more than permitted, may interfere with the system.
In the telemetry system all cables are planned so that attenuation in them
equals to the amplifiers on the antenna plates. Do not use any other cables, or
shorten or extend the cables.
Do not use hypochlorite, acetone-, phenol- or ammonia-based cleaners.
Do not autoclave the device or its parts.
Do not immerse any part of the device in liquids, or allow liquid to enter the
interior.

The Datex-Ohmeda S/5 Arrhythmia Workstation is a monitoring central for


analyzing ECG signals sent from the S/5 Critical Care monitors, C/3 monitors or
Cardiocap/5 monitors (or from the patient transmitters if telemetry option is
used).
The ECG signals are analyzed and all the events are stored on a hard disk. If full
disclosure option is used, the actual ECG signals are stored for up to 24 hours.
Concurrently with the recording of new ECG waveforms, the ones that are more
than 24 hours old are discarded.
The smart key determines the number of beds that can be configured in the
system. There are keys for 4, 6, 8, 10, 12, 14, or 16 beds. The maximum
number of telemetry channels is 12 with the 14 or 16 bed configuration.
Using the Datex-Ohmeda S/5 Network and Centrals (or Information Center)
more monitoring beds are achievable to the system.

Telemetry system
In the telemetry system, the ECG signal is measured with two leads, and sent
from the portable transmitter to antennas, and linked further to the Arrhythmia
Workstation Computer. In the computer, the converter board processes the
signal suitable for the receiver, and then the signal is analyzed for arrhythmias.
See Figure 2-2.

CAUTION Operation of the telemetry system is based on radio transmission.


Electromagnetic emission from other radio transmitters or electronic
equipment emitting noise more than permitted may interfere with
the system.
If the monitoring is impaired because of the transmission interference, the
Arrhythmia Workstation will give an alarm.

Telemetry transmitter
The transmitter transforms the measured analog signal to digital, and sends it to
antenna system using the black lead wire as antenna.
The carrier wave is either 210-230 MHz (VHF) or 430 -470 MHz (UHF), and it is
modulated, using the binary phase shift keying (BPSK).
Each transmitter’s signal’s bandwidth is 25 kHz. All the transmitters in a
telemetry system must be within 1 MHz range. This is adjusted by the
manufacturer, and cannot be changed in the field.
NOTE: The transmitter cannot be switched on or off. It is always on when
functioning batteries are inserted.

Antenna plate
The telemetry antennas link the radiowaves from the transmitter to the
Arrhythmia Workstation Computer.
When a patient carrying the transmitter walks around in the ward area, the
signals will reflect from the walls, floors and ceilings causing fading.
On an antenna plate, there are two whip antennas, which are placed with a
quarter of the wavelength from each other, and each with a separate amplifier.
When the signal is weak or put-phased at one antenna, it will be at it’s
maximum at the other antenna. Because of this dual system, two signals are
sent from the antenna to the computer.

Antenna system
The antenna system consists of maximum of 12 antennas, arranged in 1 to 3
branches with maximum of 5 antennas in each, see Figure 2-5. Between the
branch and the computer an adder/divider is needed for each of the signals.
Each antenna amplifies the signal to the extent of the attenuation that takes
place in the cable. Thus, other cables but the specified ones must not be used.
Accordingly, the specified special terminators must be used in every input
connector not used in the antenna plates or adder/dividers.

Parts installed in the computer

Converter
The Frequency Converter Board, B-RFC, has a common front end diversity UHF
and VHF converter which converts the carrier waves (operating within the
frequency bands officially recognized for telemetry transmission in each
country) to lower frequency for the receiver board.

Receiver
The Receiver Board, B-RC4, B-RC3 or B-RC2, receives signals from the
converter. The number in the board’s name tells for how many patients the
receiver is configured. Up to three receiver boards can be installed into the
computer.
Because of the dual antenna system all the signals are doubled. The receiver
receives the two signals (A and B), and selects the better one by comparing the
noise level. The received data packages are opened and possible transmission
errors are handled. Then the data is passed on to the computer CPU for
arrhythmia analysis.

Data transfer
Various steps have been taken to prevent errors during transmission.
In addition to the digital data, each transmitter sends continuously its own ID-
integrity code. If the ID code is lost, a system alarm will appear.
The information sent is packaged into a matrix:
Data1 ID CRC1
Data2 ID CRC2
Data3 ID CRC3
Data4 ID CRC4
Data = measured data
ID = transmitter ID
CRC = cyclic redundancy check
The signal carries the information: Data1 Data2 Data3 Data4 ID ID ID ID CRC1
CRC2 CRC3 CRC4 ... If any of the data is lost on the way, it can be calculated with
the help of CRC.

Cyclic redundancy check


Each ECG channel is organized in blocks, and each data block is divided by a
polynomial leaving a remainder. The remainder is transmitted along the data
block. When the signal is received, the data block is divided with the same
polynomial again, and remainders are compared. Any difference indicates an
error, and thus, the corrupted bits are discovered, and original ECG reproduced.

Interlacing
Interlacing means that the transmission of one data block is spread over time.
This reduces the probability that all bits are damaged.
A brief disturbance affecting a number of consecutive bits will therefore have no
significant effect on the system’s possibility of reproducing an ECG signal,
because CRC is capable of correcting bit errors.

Binary phase shift keying (BPSK)


The transmitter uses the BPSK modulation principle which doubles the
transmission capacity compared to systems using traditional FSK (Frequency
Shift Keying) modulation.

3 INSTALLATION

Unpacking
1. Ensure that the packing box is undamaged. If the box is damaged,
contact the shipper.
2. Open the top of the box and carefully unpack all components.
3. Ensure that all the components are undamaged. If any of the
components is damaged, contact the shipper.
4. Ensure that all the components are included. If any of the components is
missing, contact your Datex-Ohmeda distributor.

The Arrhythmia Workstation is preinstalled in factory, all you need to do is:


1. Plug the display, mouse, and keyboard together, and connect the
loudspeakers.
2. Insert smart key into the parallel communication port.
3. Turn the power on.
The smart key sets the number of monitoring places that can be attached to the
workstation and patient sectors displayed on the screen.

Software is pre-installed in the factory.


If reinstallation is necessary:
1. Start the workstation computer.
2. Quit the arrhythmia software by pressing Ctrl-F2 and giving the
password DADE. Then you should get to operating system level.
3. Insert the installation diskette into the drive A.
4. Write A:\Install and press Enter.
5. Follow the displayed instructions. See chapter 4 Configuration for
instructions on how to edit Config.doc file.

Arrhythmia Workstation functional check


Configure the system before performing the functional check, see chapter 4
Configuration.
1. Start up the system.
2. Start up all the monitors that are installed in system and connect them to
simulator sending ECG waveform.
3. Check that the waveforms and digits from all the monitors are displayed
on the Workstation screen
4. If problems occur, see the chapter 7 Troubleshooting.

Cabling
Cabling depends on the system you have, materials used etc. Refer to Datex-
Ohmeda S/5 Central and Network Technical Reference Manual.

Telemetry installation
NOTE: Before building a telemetry system you must have a license from local
authorities to use certain channel of VHF or UHF radiowaves. Inform your local
Datex-Ohmeda distributor of this channel, the telemetry transmitters are made
accordingly.
After hardware installation, perform the configuration and functional check.

When the telemetry option is purchased, the required boards (converter and
receiver boards) are pre-installed. However, it may be necessary to add more
receiver boards.

Adding a Receiver Board


It is possible to install up to 3 receiver boards to the system. Each board must
be addressed uniquely using the dip switches.

Installing the board


NOTE: Make sure your workstation is properly grounded, and wear a wrist strap
connected to ground. Hold the board by the edges only. Avoid touching the
board elements or gold connectors.
1. Remove the top cover of the computer and select the suitable slot.
2. Remove and save the expansion slot cover and screw.
3. Remove the board from its wrapper.

4. Ensure that the dip switch is set correctly (see Table 1), and that there is
only one board with each address (1, 2 or 3).
5. Press the board into the appropriate expansion slot.
6. Attach the board with a screw. Make sure that the bracket slot is well
against the screw, otherwise the bracket may interfere with an adjacent
bracket.
7. Connect the two cables from the receiver to the converter board
connector. Make sure they are in adjacent positions.
8. Terminate the empty connector positions with the converter terminators.

Installation

Dip switches
The dip switches must be set uniquely for each receiver board in the system to
address it right. Switches on the receiver board can be adjusted e.g. with a
small screwdriver through the three holes in the covering plate. The stickers
nearby show the correct switch positions, and a small round sticker shows the
card’s factory settings.
NOTE: Only Receiver boards 1, 2 or 3 can be used.

Antenna installation

Antenna coverage
The thumb rule for planning the antenna system is that one antenna covers an
area of 30m x 30m. This can be used for approximating the need of antennas in
a system.
The final antenna density depends on the environment, e.g. factors listed in
Table 2. The simple antenna system, where the antenna functioning radius can
be up to 20 meters, can be used with good radio transmission conditions with
minimum disturbance. In difficult conditions intensified system is required, e.g.
one antenna coverage radius being 15 m.

Maximum antenna system


Maximum system consists of 12 antennas, arranged in 1 to 3 branches with up
to 4 antennas serially in one branch. The antenna system solution can vary
depending on the shape of the area and environmental conditions.

Planning the antenna installation


1. Obtain a floor plan of the area to be covered.
2. Draw the position of the workstation.
3. First place an antenna 15 m from the most distant wall of the area to be
covered.
4. Design the place for the next antenna towards the central unit.
Remember that the cable length (30 m) cannot be altered, though the
cabling channels may not be direct.
5. Repeat this procedure until entire area has been covered.
6. Place the adder/divider so that the distance from all the branches are
within reach of one cable length (30 m).
7. Check that the adder/divider is placed so that it can be connected to the
Arrhythmia Workstation with a 4-meter cable.
8. Contact a Datex-Ohmeda Arrhythmia Workstation specialist for checking
your plans.

CAUTION All cables are planned so that attenuation in them equals to the
amplifiers on the antenna plates. Do not use any other cables, or
shorten or extend the cables.
Consider the factors listed above in Table 2 whether simple or intensified
antenna system is needed.

Example:
Antenna system should cover an L-shaped department with 6 patient rooms
along the corridor, see Figure 3-7 and Figure 3-8.
Figure 3-7 shows an example of a simple system where radio transmission
conditions are good and disturbances minimal.

Figure 3-8 shows a plan for place, where hinders (thick walls etc.) or
disturbances (MRI, electrosurgery etc.) force to use an intensified system.

Installing the antenna system


Use only the cables shipped with the antenna plates and adder/dividers. Do not
shorten or extend the cables. The amplification of the components is
dimensioned for these cables only. Using other cables may impair or restrain
the transmission of telemetry signals.

1. Start with the antenna at the most distant end.


2. Attach the antenna plate to the ceiling with appropriate mounting
elements. E.g. place the plate above the ceiling panel and drill two holes
so that the whips can protrude.
3. Attach the special terminator in the input (EXT).

NOTE: Use only specified terminators. Other terminators may destroy the
system.

4. Attach the next antenna less than 30m from the first one towards the
workstation.

5. Connect the two cables from A and B output connectors to A and B


inputs of the next plate.
6. Repeat the steps until all the planned antennas are attached.

7. Connect the two cables from A and B output connectors of the last
antenna plate to an adder/divider.

8. Connect the cables from adder/dividers to the converter board.

9. Attach the special terminator to unused connectors


10. Install other branches.

Functional check of the telemetry system


Before the functional check, the system must be configured, see chapter 4,
Configuration.

Checking the antenna system


Check the antenna system with minimizing the transmission capability of a
transmitter.
1. Make sure you have batteries in the transmitter.
2. Wrap the black lead wire (transmitter’s antenna) around the transmitter.

3. Admit this transmitter as “patient” in the workstation.


4. Check that the workstation receives the signal (a message ‘Leads off’ is
displayed) from every point of the covered area. If the message ‘No ECG’
appears, the signal is missing.
5. If the signal fades an antenna must be added. Before that check also
chapter 7, Troubleshooting

Checking the entire telemetry system


1. Start up the system.
2. Connect all the transmitters to ECG simulators.
3. Check that all the patient sectors are named, as they should, and that all
waveforms and digits are displayed on Arrhythmia Workstation screen.

4 CONFIGURATION

Steps of configuration
1. Start the workstation computer.
2. Go to Normal Screen and make sure all menus are closed.
3. Quit the arrhythmia software by pressing Ctrl-F2 and giving the password
DADE, so that you are in operating system level.
4. Edit the Config.doc file.
5. Save the file, and take a back-up copy of it.
6. Write INIT and press Enter to initialize the system.
NOTE: Initialization clears all the patient data from the workstation’s
memory.
7. Reset the monitor.

Editing Config.doc
NOTE: Make sure you configure each monitor to one Arrhythmia Workstation
only.
Make sure you write the correct number of characters and spaces to the exactly
correct position. The #### string shows the place and maximum length of the
item. See the examples below (written in COURIER typeface).
1. Enter the name of your Server name in S/5 network (max 39 characters)

Arrhythmia Workstation Computer, C-ARRWS and C-ARRWSTEL

C-ARRWS is a Pentium computer for Arrhythmia Workstation without telemetry


option. The computer has all the necessary boards installed (display controller,
network and sound blaster boards.)
C-ARRWSTEL is a Pentium computer for mixed system with both telemetry
transmitters and S/5 monitors. The computer has the display controller,
network and sound blaster boards. If the telemetry option is purchased the
receiver and converter boards are installed as well.
NOTE: The C-ARRWS cannot be updated to functions as C-ARRWSTEL.

Patient telemetry transmitter, T-PE2


Two ECG signals are continuously transmitted from the patient to the central
monitor system while the patient can move within the area covered by the
antenna system. In addition to ECG signals, the messages created by pressing
the PATIENT MARK and NURSE CALL keys are sent.

Functional description
The electrode disconnection (‘Leads Off’) is monitored by feeding a bias current
through the electrodes.
The ECG amplifier ensures that the ECG signal is on an appropriate level for
further sampling and transmission. It also includes overload detection and
active recovery.
After amplification there is a pace maker pulse detection circuitry. It detects
pacer spikes by the shape of the signal.
Analog to Digital Converter takes samples 200 times/second and converts
every sample to a number with 10-bit resolution.
The signal is further processed and modulated for safe and fast transfer (see
chapter Data transfer).
High frequency transmitter is designed for low power consumption.
Transmission is modulated by the BPSK signals. The black lead wire works as an
antenna.
The power supply works with two 1.5 V batteries.

Telemetry Antenna, N-API

Functional description
An antenna plate consists of two whip antennas each having its own amplifier
on an aluminum plate.
Antenna whips are placed 1/4 wavelength from each other to enable that when
a wave is weak or put-phased at one whip, it will be at its maximum in another.
This is called true diversity, which also means that two signals (A+B) are passed
to the converter and receiver for each receiver channel.
The power for amplificators is supplied from the converter board in the computer
via the same cables as the signals but to the opposite direction.
The amplifying capacity is equal to attenuation taking place in the cables. Thus,
the cables must not be shortened or extended, and no other cables must be
used.
The connector(s) that are not used for cabling must be terminated with a
specified special terminator. The terminator is not a resistance only but has also
a condensator.

Frequency Converter Board, B-RFC

Functional description
When a signal is received from the antenna, it will first be amplified in a LNA (low
noise amplifier).
After the amplifier has passed the signal through an image filter, it is mixed
down to 43-44 MHz. After that there is another amplifier with an image filter. In
the last part of the image filter there is a divider used to distribute the signals to
the receiver boards.
The two receivers (A and B) join the same injection signal. The signal is
generated by an oscillator, and a multiplicator multiplies the frequency by 3. A
buffer is placed after the multiplicator. In the VHF version this buffer is a normal
amplifier, while in the UHF version it multiplies the frequency by 2.

The number in the board’s name refers to the number of channels received.

Functional description
The signal comes from the converter board output and goes into the two splitters
that divide the signal into 2 times 4 identical signals.
From the splitter, the signals go to diversity intermediate frequency receivers
that transmit a total of 4 differential BPSK signals. The BPSK signals are
converted into a digital bit flow in 4 separate demodulator circuits.
Then the signal goes to a microprocessor that unpacks data and corrects errors
that may have occurred during the transmission. After error correcting and
unpacking, the signals are transmitted to the central processor of the PC via the
PC bus.

6 CLEANING AND MAINTENANCE

Cleaning
The Arrhythmia Workstation computer and transmitters can be wiped with soft
moisturized cloth.

CAUTION Do not use hypochlorite, acetone-, phenol- or ammonia-based


cleaners.
Do not autoclave the device or its parts.
Do not immerse any part of the device in liquids, or allow liquid to
enter the interior.

Preventive maintenance
Perform following maintenance procedures at least once a year.

Computer and network


1. Check that all the components of the workstation are clean and
undamaged. Make sure that the cooling holes are not obstructed.
2. Check that all cable connections are secure, and that no cables are
pulled out. Make sure that the wallboxes are clean and undamaged.
3. Check that the system starts up normally, and that all active devices
connected to workstation register to it.
4. Check that the display contrast and brightness are adequately adjusted,
and that the display picture is placed in the center of the screen. For
readjustments, please refer to the computer’s user documentation.
5. Check that the keyboard keys function properly by pressing the keys one
by one.
6. Check that the clock shows the correct time. For readjustments, please
refer to the computer’s user documentation.
NOTE: Use the time displayed on the monitor. Networked monitors and
Central are synchronized but the Arrhythmia Workstation is not.
7. Make a back-up file of your config.doc file.
8. Run an appropriate disc-scanning program. For further instructions,
please refer to program’s user documentation.

If the workstation is located in the area where it is suspected to collect dust, it is


recommended to open the system unit cover and to remove dust that might
have accumulated around the power supply unit. For preventive maintenance of
printers, power supplies, and hubs, please refer to the corresponding user
documentation.

Telemetry transmitter
1. Check the transmitters visually.
2. Check that all the transmitters in the system function as they should, see
Functional check of the telemetry system.
3. Check and clean the battery cabin: open the battery cabin, remove the
batteries and wipe the patient connectors with soft cloth.
