1.0 Purpose
This procedure specifies actions to be taken in case of situation defined as an
emergency.

2.0 Scope
The guidelines issued are applicable to all the staff that may find themselves in an
emergency.

3.0 Reference
All the point of an emergency will be referred to the Security PIC whom is responsible
for the preparation and issue of the emergency plans. They should be clearly displayed
where they are likely to be required or easily available.

4.0 Definition
A serious situation or occurrence which happens unexpectedly and demands
immediate action. A condition of urgent need for action or assistance: a state of
emergency. In the event of an emergency safety is an over-riding priority for all
personnel both in physical and psychological terms. In all cases personnel are
requested to maintain a calm composure and re-act as planned to overcome such
situation. We has classified the below situation as an event of emergency.

a) Fire Situation.
b) Electrical Failure blackout plant wide.
c) Access Control System Fail.
d) Hostage or Bomb Threat.
e) Chemical spill & Guideline.
f) Work Related Accident / Medical Emergency.
g) Handling H1N1 Outbreak situation.

6.0 Procedure Flow and Detail


6.1 Fire
6.1.1 If in case fire is discovered, a sound of the fire alarm will be triggered and it’s advisable to use an alarm button. If it is
possible without taking risk to self or others, attempt to use extinguish the fire with available equipment. Do not attempt
this unless you are trained and competent in firefighting techniques.

6.1.2 Upon hearing the fire alarm all staff should exit the building using the nearest fire exit.

6.1.3 Evacuate the building in an orderly manner by using the nearest Exit and proceed to the designated Emergency assembly point.
(Appendix 3). The Emergency Door Lock should release automatically when the fire alarm triggered. You should unlatch the door
latch to exit the Emergency Door. In any case an Emergency door does not open, do not hesitate to use the Emergency Release
Break Glass next to the Emergency Door and force “Break Glass” to open the door to gain access-controlled doors.

6.1.4 Once outside the building, place yourself at “Assembly point” (Appendix 3) and be advise strictly that do not under
any circumstances re-enter the building until the emergency is over.

6.1.5 Supervisors in charge of the various departments would assembly lines of their people to conduct a roll call to
ensure their people are verified safe. Any absences are reported to the Security PIC and team immediately.

6.1.6 Safety Committee or Personnel trained in first-aid should make themselves available to assist to find the missing or injured
personnel at the scene immediately.

6.1.7 The BOMBA services should be called automatically by the remote Continuous Monitoring Station. The BOMBA
services team will contact to ascertain the fire outbreak. IF not, the Security PIC or Safety Officer should contact
the emergency services thru speed dial in any telephone by dial 013. By ready to give the following information:
 Where the emergency is located and required.
 Relevant details for the emergency situation.
 Your name and contact number.

6.1.8 The Security PIC will post guards or volunteers at a safe distance to all fire exits and the perimeter to make sure
there is no attempt to enter or re-enter the building other than by train members of the emergency services until the
emergency are over, and the all-clear call is given. The Security Manager and guards must be in visual contact. In
the event of the Security PIC being unable to perform this event during the emergency, the shift supervisor will
assume as a senior responsibility to run the situation.

6.1.9 Once the emergency is over and the all clearance is stand-down by Security PIC
or second in command (Chief Security Officer Supervisor), All personnel can
re-enter the building via the main entrance using standard entry Protocol which
will be monitor by Security personnel. In production/Store Employees shall ensure
work in progress must have a complete recount for their inventory in-tact and all
secure trolleys must be examined to ensure no tampering has occurred. Once the
recount has occurred and there are no problems, planned work may commence as usual.

6.2 Electrical Failure or Blackout Situation


6.2.1 In the event of an electrical mains failure the Security PIC or Facilities Management
representative must report the failure immediately to the electricity supplier
TNB.

6.2.2 The security access control system and emergency lights has a battery backup
electrical supply of approximately 7 hours that will come into effect in the event
of the mains electricity supply failure. If in case, an electrical supply is not
connected by TNB within time frame of 4-6 hours, the management would then decide
on the employees whether to retain them for said hours or let them off duty in
management description.

6.2.3 In the event of total electrical failure (mains and battery) affecting the security
system, the Security PIC/supervisor must appoint guards to control access and
directing an employees to emergency assembly areas for safer situation. No one
shall be allowed to enter or re-enter the company till situation is declared call
off. All at said premised will be on authorized personnel only.

6.3 Access Control System Failure


6.3.1 In the case of access control system failure, the Security PIC or Security Officer
would rapidly checks the system for faults and attempts to reboot/reactivate the
system according to the supplied training and documentation for the system.

6.3.2 If the system still cannot be activated the Security PIC or security e must
immediately call the supplier for urgent technical support.

6.3.3 While the system is non-functional the Security PIC must post guards at the most
critical areas including the production area doors and the vault entrance. The
Security PIC must prioritize the re-establishment of the access control system
to full functional order. In performing this priority, the shift supervisor must
be available to command supervision of posted guards.

6.3.4 Once the Security PIC confirms the access control systems are fully functional
and operational, normal status may resume.

6.4 Hostage/Bomb threat
6.4.1 Remain calm and refer to the available bomb threat checklist or hostage situation
checklist (Appendix 4 and 5 )

6.4.2 If the threat is within the building(s) should be evacuated provided there is no
external threat by the safest and fastest route based on information available
at the time.

6.4.3 If the threat is external to the building and evacuation could endanger staff then
they should remain within the building as far from the suspected source of threat
as possible.

6.4.4 Ensure that personnel stay clear of any area that may contain a threat until the
emergency is over and the all clear has been given by the emergency services.

6.4.5 With all threats contact the police immediately by calling Speed dial 014 or dial
999.

6.5 Chemical Spill Procedures


6.5.1 Procedures for Chemical Spill
This procedure outlines the requirements for the management of chemical spills
in the workplace to minimize effects to health and safety from exposure to chemical
spills and reduce the impact on the environment. The procedure applies to all staff
and contractors in all areas of the plant premises where chemical substances are
transported, purchased, stored, handled, or used, including vehicles of visitors
or suppliers who bring substances into the company that are potentially hazardous.

The procedure provides general guidance and therefore each area that use chemical
substances should have specific procedures for the particular types of substances
used within the workplace or brought onto the premises.

6.5.2 The following procedures should be followed in the event of chemical exposure.
In all cases, the incident should be reported to your Safety & Security PIC, Security
Officer and Supervisor regardless of severity. Consult your department manager
before First Report of Accidental form as attach in “T” Drive complete and sent
it to them for review and vet. This report shall reach Safety department by 48
hours after incident occurred.

6.5.3 Immediate Actions upon spill detection are as follows


The primary concern upon chemical spill is to protect and ensure human health and
safety is protected, therefore act in accordance to the steps as follows:-
1. Clear the affected area
2. Check for any persons involved

3. Personnel contaminated with chemicals must be decontaminated for at least 15 minutes (emergency shower)
and taken for medical examination
4. Administer first aid
5. Isolate the spill (if safe to do so)
6. Contact the Safety PIC .
7. Gather any information possible, i.e. identify the material and quantity, gather relevant MSDS and assess any
immediate risks.

6.6 Work Related Accident/Medical Emergency


6.6.1 If the employee or the supervisor believes there is a need for emergency medical treatment, a call for emergency medical
treatment must be made immediately. Always error on the side of caution even if the injury may not appear serious.

6.6.2 After calling the emergency services, notify the security guard so that immediate access can be made in line with
the section below Admitting Members of the Emergency Services.

6.6.3 Out of hours it may be necessary to admit emergency services via loading bay, or fire door. Notify external security
monitoring agency in the case of the latter. The supervisor should place a guard on any door open to the outside.
The guard should admit only emergency service personnel.

6.6.4 If the injured person needs immediate medical treatment but not emergency treatment, the supervisor must arrange
to transport the employee to the nearest treatment centre.

6.6.5 If the injury is minor and only requires first aid, first aid must be provided.

6.6.6 If the employee declines medical treatment and the supervisor feels that medical treatment is necessary, the
supervisor may direct the employee to obtain medical treatment.

6.7 Handling out Break of H1N1 Emergency situation


6.7.1 This policy is attached in the (T Drive Folder 13-Security folder for immediate action).

6.8 Handling of any sort Emergencies as explained in Definition page 1 required following actions.
6.8.1 Any employee under coercion to enter their code to disarm the alarm must input a confidential alarm code for this type of
situation. The details of this code and the nature of its operation will be explained to authorized alarm code holders only. It
is the function of the Security PIC to perform this task.

6.9 Reporting
6.9.1 In the event of any emergency, the most senior member of management present must
fill out a detailed incident report (Supervisor’s Incident Investigation/Report
of Accident or Injury-Form A). CAR may be raised as necessary to initiate root
cause investigation and corrective and/or preventive actions.

6.10 Description of Role Responsibilities for Crisis Response & Emergency Response Team

Note: Not all Crisis or Emergencies Team Roles will be necessary for each crisis
or emergency. An individual person may have multiple roles and function with
competent requirements.

6.11 Maintenance / Inventory for ERT Team
Emergency equipment must be maintained, inspected and tested to maintain proper
working order as part of preparation and planning for an emergency. In the event
of an emergency, properly functioning emergency equipment can help minimize both
personal injury and property damage. Types of emergency equipment include eyewash
stations, safety showers and fire extinguishers.

6.11.1 Eyewash Stations and Safety Showers


It is extremely important to maintain eyewash stations and safety showers. They
should be inspected and flushed weekly to ensure proper working order. Weekly
flushing not only ensures that they are functional it helps guard against several
contaminants that can cause damage to your eyes and/or compromise your health.

6.11.2 Fire Extinguishers inside buildings are inspected Quarterly by One LAX Capitol
& Four Up Company. Trained competent personal are familiarize with the location of all fire extinguishers in our
company. Fire extinguishers can be used to extinguish small fires if you can do so without putting yourself or co-
workers in danger. Although, improper use of fire extinguishers can potentially exacerbate a situation. For that
reason, it is essential that you receive training before using any fire extinguisher.

6.11.3 All Emergency Inventories/Maintenance item shall be able to get it from Production Floor, Safety Office or Security Office which
would be accessible for emergency situation and all this maintenance record is maintain by First Aider team as refer in Table 1
below. All other item related such as Hose, sprinkler, Fire Extinguisher, and Alarm panel are maintained by Fire Contractors
engaged by the plant and a copy will be handled to Safety & Security Department for Record purpose.

6.12 Emergency Response Procedure Testing & Review


The emergency response procedures will be periodically tested where practical. The procedure will also be
reviewed after the actual occurrence of an emergency to identify any potential improvement needs. The incident
commander is responsible to coordinate a post mortem meeting after incidents to review the response procedure.
