1. Discuss the role of Nurses in Health Assessment Process.

- A nurse takes note of actual or potential problems her patient may have during a health
assessment. The important role of nurses in health assessment is that they collect comprehensive
data related to the patient’s health or situation. From the list of problems, the nurse formulates
diagnoses, which they use to create a nursing care plan.

2.    How the health assessment become an essential nursing function which provides
foundation for quality nursing care and intervention?

- The purpose of a nursing health assessment is to collect holistic, subjective and objective data to
determine a patient’s overall health and well-being to establish a professional clinical judgement.
Health assessment is to make judgement or diagnosis because health treatments and decisions are
based on the data gathered during the assessment. It is the outmost importance that the assessment is
factual and complete, providing the basis for clinical decision-making. Overall, assessment is a way
of looking broader into a patient’s illness and preventing more problems from occurring.

3.    How the health assessment helps identify the strengths of the clients in promoting health?

- Health assessment helps nurses to identify the strengths and weaknesses of their clients’ health.
Particularly, it enables to identify areas that might require health promotion as well as what might be
done to improve patient’s health.  By doing so, this helps the nurses to make good judgments
regarding the type of care or intervention that should be offered to different patients. 

4.    How the health assessment helps identify client’s needs and clinical problems?

- The gathered data information by the nurse provides a comprehensive detail of the patient. It
focuses on the patient’s needs at that particular moment in time and the possible needs that may need
to be addressed in the future. The help of nurses through health assessment are able to identify the
medical conditions of patients and the right intervention for those patients.
5.    How health assessment reflects the skills, knowledge and attitude of a professional nurse?

- When nurses are conducting a health assessment on a person it requires knowledge and techniques
of collecting and analyzing subjective and objective data to include both what the patient says about
themselves regarding their health and the physical assessment that was observed by the nurse from
inspecting, percussion, palpation and auscultation during the physical examination. The duty of a
professional nurse is the one who puts the needs and importance of patient care above all
others. When a nurse has established a relationship with their patient, they must be careful not to
show any judgment on a patient's decisions, beliefs, values, or culture. With the help of teamwork
along with the physicians, the nurse is able to conduct a successful assessment.
