Discuss the role of nurse in health assesment


- Health assessments are performed by nurses in a variety of settings, including at health fairs, in
the community, and at schools and hospitals. It involves interviewing a patient, inspecting and
observing her body and actions, and palpating and percussing parts of her body. There are six
roles of nurse in health assesment

1.Information - This information is used to formulate a nursing plan of care for the patient.

2.Nursing Diagnoses and Care Planning - A nurse takes note of actual or potential problems her
patient may have during a health assessment. From the list of problems, she formulates
diagnoses, which she uses to create a care plan.

3.Managing Problems - The nurse continuously does a health assessment on her patient to see if
her care plan is having the desired effect. If not, she makes changes to her care plan to address
the patient's health problems.

4.Evaluation - Evaluation of a patient's health status is done through health assessments.


Evaluations determine if a patient has responded to nursing care sufficiently enough to be
recommended for discharge.

5.Discharge Teaching - During a health assessment, a nurse may become aware that a patient is
lacking information that may help improve his condition. This provides the nurse with an
opportunity to impart this information before he is discharged.

6. Advocate - In this case, the nurse notifies the proper health care team member of the problem
and makes sure the patient receives the expert care that they need. Here, she becomes an
advocate for her patient.
2. List and explain the types, methods, techniques, components of Assessment.
- Techniques:
1. Inspection - is a visual examination of the patient; palpation is done when the person doing
the assessment places their fingers on the body to determine things like swelling, masses, and
areas of pain.
2. Auscultation - assesses airflow through the trachea-bronchial tree. It is important to
distinguish normal respiratory sounds from abnormal ones for example crackles, wheezes, and
pleural rub in order to make correct diagnosis.
3. Palpation - involves evaluating the health of a person's muscles and other soft tissues by
touching these muscles with the fingers and hands.
4. Percussion - is a method of tapping on a surface to determine the underlying structures, and is
used in clinical examinations to assess the condition of the thorax or abdomen. It is one of the
five methods of clinical examination, together with inspection, palpation, auscultation, and
inquiry.
Methods:
5.Interviewing - Collecting patient data is a core step in the nursing process. Often referred to as
a nursing health assessment interview, nurses—and nursing students enrolled in nursing
programs—must systemically collect patient health information so patients can receive the care
they need.
6. Obeserving - a systematic process to collect data about a patient. This data provides
information about the patient's condition, and is used to inform the care which is appropriate for
that patient
7. Examining - allows the nurse to obtain a complete assessment of the patient.
Observation/inspection, palpation, percussion and auscultation are techniques used to gather
information. Clinical judgment should be used to decide on the extent of assessment required.
Types of Assessment:
Objective Data
This is the information that we can gather using our 5 senses. It is either a measurement or an
observation. Temperature is a perfect example of objective data. The temperature of a person can
be gathered using a thermometer.
Subjective Data
This is where we all get ourselves into trouble. Subjective data is gathered from the patient
telling you something that you cannot use your five senses to measure. If a patient tells you they
have had diarrhea for the past two days, that is subjective, you cannot know that information any
other way besides being told that is what happened. Pain is subjective because the patient is
telling you what their pain is.

nursing care and intervention. It helps to identify the strengths of the clients in promoting health.
Health assessment also helps to identify client’s needs, clinical problems or nursing diagnoses
and to evaluate responses of the person to health problems and intervention.
4. How health assessment helps identify the strengths of clients in promoting health.
- Health assessment is a systematic, deliberative and interactive process by which nurses use
critical thinking to collect, validate, analyze and synthesize the collected information in order to
make judgement about the health status and life processes of individuals, families and
communities.
