OLD SAFETY PROGRAM

Incident Report

Incident reports indicate the accidents that happen in the company’s history. These are
thoroughly investigated to ensure the person involved in the accident to be safe and be given first
aid medication as much as possible.
The workers should also comply with the management to provide sufficient information
on the company that would help improve the safety management on the company and to stabilize
the worker’s health condition as much as possible.
The company’s incident report holds the most crucial accidents that happened in their
history. 2019 is the company’s most active year since they have a lot of projects that require
safety protocols. It is also the year that they experienced major accidents in the company.

Fig. 1 shows the yearly accidents that occurred in the company’s incident report.

The company had one accident in year 2014 due to an employee being lacerated by a
sharp part of an aluminum while fabricating. The person involved required urgent medical
attention due to profuse bleeding that would be fatal if left unattended. Luckily, the worker was
sent to AUP Medical Center and was given first aid medications that would stop the bleeding the
worker had 8 stitches in his right hand. Years 2015 and 2016 were the best but also the worst
years that the company had. It is the best years because there are no accidents that were reported
in those time, but it is also the worst because the company has minimal projects that consisted
only of glassworks and welding. Year 2017 had only one accident in which the person involved
was a company driver that delivers construction equipment on the construction site. The
company driver bumped the vehicle whilst traversing the way towards the construction site
which resulted in the driver having contusions on parts of his body which Year 2018 has no
accidents although there are reports of minimal injuries such as ergonomic hazards of workers
wherein some workers strained their body too much while working on site. Year 2019 has two
incident reports that involves a person being cut by a hand grinder while doing work resulting in
small lacerations in the worker’s hands that required first aid medications which stopped the
bleeding immediately. The other accident occurred while the worker was lifting heavy objects
wherein he sprained his knee. The person involved needs to rest for 1 week and was required to
apply hot compress on the affected knee to reduce the pain.
