2. Incomplete application form is liable to be rejected. Overwriting or erasing the date of birth will not be
acceptable.
3. The Rules & Regulations of the college must be read carefully before applying for admission.
a. Application Form, complete in all respect must be accompanied by the following documents:-
i. Four latest passport size photographs of the candidate duly attested by the Head of the Institution.
ii. Bank Challan (NBP) / Bank Draft of Rs. 1000/- (If the Prospectus/ Application Form has been purchased from
the college Office or Bank) and Rs. 2000/- (If the Prospectus/ Application Form has been downloaded from
www.cccss.edu.pk) drawn on Principal Cadet College Choa Saiden Shah Chakwal.
iii. Undertaking given below must be signed by the candidate and countersigned by the Parent/Guardian.
iv. Attested Photocopy of Candidate’s B-Form.
v. Attested Photocopy of Father’s CNIC.
vi. Address slip must be completed and sent with Admission Form.
vii. Call letter must be completed and sent with Admission Form.
viii. Attested Photocopy of 10th class result card with minimum 80% marks.(Only for Class XI)
ix. If SSC Part-II result is not declared by the Closing Date, please also attach:
1. Hope Certificate (in original) by the Principal of the School of the last attended showing the fact that
the candidate will hopefully get 80% or above marks or minimum.

b. Application Form must be sent through registered post or TCS and addressed to the Principal, Cadet College

c. The Photostat application form will not be entertained.
d. Incomplete Application Form in any respect is liable to be rejected without any notice.
