parties involved in actions before the court. How: The statement of the person is either written or typed
onto the form. The person making the statement signs the statement under oath before a judge, court
clerk, or notary public. Copies: The original affidavit is stored in the applicable court case file. A copy is
given to the person making the affidavit and all other appropriate affected parties
