JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

            WE, (NAME OF AFFIANT-1) and (NAME OF AFFIANT-1), both of legal age,

Filipino, and with respective residences at (Place of Residence of Affiant-1), and (Place of
Residence of Affiant-2), after having been sworn to in accordance with law, hereby depose and
state: THAT –

1.      We personally know (NAME OF SUBJECT PERSON), of legal age, Filipino, married

to (Husband/Wife of Subject Person), and with residence at (Place of Residence of Subject
Person), being her closest friends for a long period of time;

2.      Unfortunately in some of her official documents her name was erroneously entered
as (Erroneous name of Subject Person), with “(Erroneous Detail)” in her first name;

3.      We hereby state under oath that the names (Name in Birth Certificate), as appearing in her
birth certificate, and (Name Known or in Official Documents), as appearing in some of her
official documents, including her school records and marriage certificate, refer to one and the
same person;

4.      We are not related by consanguinity or affinity to her;

5.      We are executing this affidavit to attest to the truth and veracity of the foregoing facts and for
whatever legal and beneficial purposes this may serve.
