Campaign to increase awareness to avoid pulling pipe when Rams are closed.

Details:
Pulling the string through closed Rams can be a catastrophic situation, resulting in
fatalities and/or major damage to equipment. The idea of this submission is to create a
campaign to increase awareness and to assure that in all rigs where IPM operates, a small
sign indicating when rams are closed should be used. This sign should be placed in the
drawworks clutch at the driller’s panel every time the rams are closed in order that
driller knows it and open them before start pulling the string. Please, see attached
presentation where this type of incident occurred in non IPM operations. This
presentation can be reviewed with all personnel in the rigs in the pre-tour or safety
meetings. Also a poster is included to be posted in the dog house in all rigs. We need to
avoid this to happen at all cost!!!!

Titulo: Campaña para incrementar alerta en no jalar la sarta cuando los arietes están
cerrados.
Detalles: Levantar la sarta con los arietes cerrados puede ser una situación
catastrófica, resultando en fatalidades y/o daño mayor al equipo. La idea de esta
sumisión es crear una campaña para incrementar el cuidado que en todos los equipos donde
opera IPM, un pequeño cartel indicando cuando los arietes estén cerrados sea usado. Este
cartel debe colocarse en el mando del malacate en la consola del perforador cada vez que
los arietes estén cerrados de manera que el perforador lo sepa y abra los arietes antes
de proceder a levantar la sarta. Por favor, revisar la presentación adjunta donde ocurre
este tipo de incidente en una operacion no IPM. Esta presentacion puede ser revisada con
las cuadrillas en los equipos durante las reuniones pre-turno y reuniones de seguridad .
Ademas se incluye un poster para ser colocado en exhibicion en la Casa de Perro en todos
los equipos. Debemos de evitar este tipo de incidente a todo costo!!!
