OF SPELLING OF OWNER’S NAME

law, hereby depose and say that:

1. On 13th of November 1998, I received my Official Receipt for the

Renewal of the Car Registration of my Yellow Toyota Corolla, with
Plate Number______, from Land Transportation Office, ______
District Office;

2. In the abovementioned document, however, the name stated therein is

incorrect.

3. The name written therein is spelled as “aabb”. It should be “aaaa”.;

4. Consequently, I hereby execute this Affidavit to attest to the truth of

the foregoing so that the Official Receipt of my Car Registration be
issued in my name will bear the correct spelling of my name as
“aaaa”.
