REITERATION ON THE IDENTIFICATION AND REGISTRATION OF

SINGLE SOURCE OF TRUTH (SST) AT THE SCHOOL LEVEL

Public Schools Division Supervisors
School Health and Nutrition Unit Personnel
Public and Private Elementary and Secondary School Heads
All concerned

1. Recapitulating the DepEd Task Force COVID-19 Memorandum No. 029 s. 2020
entitled “REGISTRATION OF SINGLE SOURCE OF TRUTH AT THE SCHOOL LEVEL”,
this Office mandates the identification and registration of Single Source of Truth (SST) at the
school level to assist in collecting data and reports from the field.

2. The SST at the school level may be composed of the following:

 School Principal or School Head,
 Health and Nutrition Coordinator,
 Clinic Teacher,
 DRRM Coordinator, or
 a Team of 2-3 School Teachers and Employees

3. Designated SST personnel of their school are expected to provide status reports,
including:
 Status of school facilities
 Status of school personnel and learners’ health, and
 Status of services rendered during the community quarantine period.

4. DepEd Mobile Monitoring Apps will be used for the aforementioned purpose. Hence,
designated SST per school must enlist at http://bit.ly/DepEdSSTRegistrationForm on or
before 12:00 Noon of April 8th, Wednesday, for pre-registration of users.

5. For immediate dissemination and appropriate action.
